摘要
目的评价压力感应导管(SmartTouch^TM导管)联合Visitag软件技术在心房颤动导管消融中的临床疗效。方法入选2015年1月至2016年6月在成都市第二人民医院心内科进行导管消融术的阵发性心房颤动患者128例,按手术顺序分为研究组(术中使用SmartTouch^TM导管联合Visitag技术,65例)和对照组(术中使用普通盐水灌注TC消融导管63例)。记录环肺静脉不同区域导管与心房壁的接触压力大小、逐点消融每点消融时间、肺静脉单圈隔离率、肺静脉-左心房传导恢复率、总手术时间、X线曝光时间、X线曝光量和术中并发症以及1年复发率。结果本研究所有患者术中肺静脉前庭隔离率均达100%o研究组肺静脉前庭单圈隔离率72.3%、肺静脉-左心房传导恢复率3.1%均优于对照组,差异均有统计学意义(P<0.05)。两组总手术时间、X线曝光时间及术后12个月随访房颤复发率比较未见统计学差异(P>0.05)。两组均有1 例并发症,研究组术后出现短暂性失语,对照组术后发生动静脉瘘,但这2例并发症与导管消融操作无关。结论SmartTouch^TM导管联合Visitag技术进行房颤射频消融术是安全有效的,可以提高肺静脉前庭的单圈隔离率,降低肺静脉-左心房传导恢复率。
Objective To evaluate the clinical efficacy of SmartTouch^TM catheters combined with Visitag technology in atrial fibrillation catheter ablation. Methods 128 patients with catheter ablation of paroxysmal atrial fibrillation in the Department of Cardiology of Second People's Hospital of Chengdu from January 2015 to June 2016 were divided as study group (using SmartTouch^TM catheter and Visitag, n=65)and control group (using TC catheter, n.=63) according to the surgical procedure. The contact force between the catheter and the atrial wall in different regions around the pulmonary vein, ablation point-by-point ablation time, the pulmonary venous isolation rate, pulmonary vein-left atrium conduction recovery rate, total operation time, X-ray exposure time, X-ray exposure, complications and one year recurrence rate were recorded. Results All pulmonary veins were isolated in both groups. The isolation rate of the pulmonary vein vestibule was 72.3% and the recovery rate of pulmonary vein-left atrium was 3.1% in the study group. That were better than the control group and the differences were statistically significant (P<0.05). There were no significant difference in the total operation time, X-ray exposure time and the recurrence rate between 2 groups. There was 1 complication in every group. Transient aphasia occurred in the study group and arteriovenous fistula occurred in the control group. However, these were not related to catheter ablation. Conclusion The use of SmartTouch^TM catheter combined with Visitag technology are safe and effective in AF ablation, which can improve the rate of unilateral isolation and reduce the recovery rate of pulmonary vein-left atrial conduction.
