血清纤维蛋白原样蛋白1与慢性乙型肝炎肝纤维化程度的相关性 被引量:3

Association between plasma fibrinogen-like protein 1 and liver fibrosis in chronic hepatitis B patients

摘要 目的分析不同程度肝纤维化慢性乙型肝炎(CHB)患者的血清纤维蛋白原样蛋白1(FLP1)水平,探讨FLP1对于评估乙型肝炎肝纤维化程度的价值。方法收集2016年6月至2017年1月在浙江省人民医院就诊并行肝脏穿刺活检的CHB患者(病理证实的轻度肝纤维化、中度肝纤维化、重度肝纤维化各14名),与14位健康对照进行临床特征、肝功能、血常规及血清FLP1浓度的比较,并分析CHB患者显著肝纤维化(中度肝纤维化+重度肝纤维化,S≥2)的独立预测因素结果血清FLP1水平在健康人与CHB不同程度肝纤维化患者中比较差异具有统计学意义(F=7.002,P<0.01)。轻、中和重度肝纤维化患者的FLP1水平分别为(73.2±17.8)ng/mL,(60.5±19.4)ng/mL和(63.1±17.2)ng/mL,均明显低于健康对照组(t=2.267,P<0.05;t=3.513,P<O.Ol;t=3.411,P<0.01)。年龄较大(OR=1.303,95% Cl:1.113~1.526)、ALT(OR=1.042,95% CI:1.012~1.074)及FLP1水平较低(OR=0.948,95% CI:0.905~0.995)为CHB患者显著肝纤维化的独立预测因素结论肝纤维化患者血清FLP1水平低于健康人年龄、ALT、FLP1是预测CHB患者肝纤维化独立因素。 Objective To evaluate the plasma fibrinogen-like protein 1 (FLP1) level in chronic hepatitis B (CHB) patients with different fibrotic stages, and explore the value of FLP1 in the diagnosis of liver fibrosis. Methods CHB patients who underwent liver biopsy (14 subjects each in mild liver fibrosis, moderate fibrosis and severe fibrosis) in Zhejiang Provincial People*s Hospital from June 2016 to January 2017, along with 14 healthy controls were collected. The clinical features, liver functions, blood routine and plasma FLP1 level were compared. The independent predictive factors of significant fibrosis (moderate fibrosis 4-severe fibrosis,S≥2) in CHB patients were analyzed. Results The difference of plasma FLP1 levels in healthy people and CHB patients with different fibrotic stages had statistical significance (F=7.002,P<0.01). The FLP1 levels in mild fibrosis [(73.2±17.8)ng/mL], moderate fibrosis [(60.5±19.4)ng/mL] and severe fibrosis [(63.1±17.2)ng/mL] patients were significantly lower than that in healthy controls (t=2.267,P<0.05;t=3.513,P<O.Ol;t=3.411,P<0.01). Older(OR=1.303,95% Cl:1.113~1.526), higher alanine ami notransferase (ALT)(OR=1.042,95% CI:1.012~1.074) and lower FLP1 level (OR=0.948,95% CI:0.905~0.995) were proved to be independent predictive factors of significant fibrosis in CHB patients. Conclusions Plasma FLP1 level is lower in liver fibrosis than in healthy people. Age, ALT and FLP1 are independent predictive factors of significant fibrosis.
