摘要
目的探讨经跗骨窦入路微型锁定钢板治疗跟骨sandersⅡ和Ⅲ型骨折的临床效果。方法选取梅州市第二中医医院与梅州市梅县区城东镇卫生院2016年6月~2018年6月收治的60例跟骨sandersⅡ和Ⅲ型骨折患者作为研究对象,随机分为对照组与观察组,各30例。对照组采用撬拔复位克氏钉外固定治疗,观察组采用经跗骨窦入路微型锁定钢板治疗。术后对患者进行3个月的随访,了解患者骨折愈合情况,并且观察患者有无切口感染与皮肤坏死的情况。同时,采用足踝功能评分系统(AOFAS)评估患者治疗后的足功能。结果观察组术后3个月的骨折愈合率为100.00%,高于对照组的66.67%,差异有统计学意义(P<0.05)。观察组的切口感染与皮肤坏死率低于对照组,差异有统计学意义(P<0.05)。观察组治疗后的sandersⅡ型、Ⅲ型患者优良率均高于对照组,差异有统计学意义(P<0.05)。结论在跟骨sandersⅡ与Ⅲ型骨折患者中应用跗骨窦入路微型锁定钢板治疗,可避免术后皮瓣坏死与切口感染等并发症的发生,值得临床推广。
Objective To explore the clinical effect of mini-locking plate through tarsal sinus approach in the treatment of calcaneal sanders Ⅱ and Ⅲ fractures. Methods Altogether 60 cases of calcaneal sanders type Ⅱ and type Ⅲ fractures who were admitted to the Second Traditional Chinese Medicine Hospital of Meizhou City and Chengdong Town Health Hospital of Meixian District of Meizhou City from June 2016 to June 2018 were selected as the study subjects and randomly divided into the control group and the observation group,30 cases in each group. The control group was treated with prying reduction and external fixation with Kirschner nail,and the observation group was treated with mini-locking plate treatment via sinus approach. The patients were followed up post-operatively for 3 months to understand the healing of fracture,and to observe whether the patients had incision infection and skin necrosis. At the same time,ankle and foot function scoring system (AOFAS) was used to evaluate the post-treatment foot function of patients. Results After 3-month follow-up,the healing rate of fracture of the observation group was 100.00%,which was higher than 66.67% of the control group,the difference was statistically significant (P<0.05). The incision infection and skin necrosis rate of the observation group were lower than those of the control group,with statistically significant difference (P<0.05). The excellent rate of sanders type Ⅱ and type Ⅲ patients of the observation group was higher than that of the control group,with statistically significant difference (P<0.05). Conclusion Application of mini-locking plate through tarsal sinus approach in the treatment of calcaneal sanders type Ⅱ and type Ⅲ fractures can avoid complications such as necrosis of skin flap and infection of incision after operation,which is worthy of clinical promotion in the future.
