摘要
目的研究过敏性紫癜(HSP)患儿链球菌感染情况,阐明儿童过敏性紫癜合并链球菌感染的临床特点。方法选取杭州市儿童医院2016年1月～2017年12月收治的HSP患儿106例。根据106例患儿咽拭子细菌培养结果,将合并链球菌感染的HSP患儿分为A组,其余均为B组。比较A、B两组患儿皮疹、关节痛、腹痛和(或)便血、便秘、肾脏损害及心脏损害百分率的差异。结果 106例HSP患儿中,根据咽试子细菌培养结果显示链球菌感染18例(16.98%),其中17例为A群化脓链球菌,1例为星座链球菌。A、B两组间皮疹、关节痛、腹痛和(或)便血、便秘的百分率比较差异无统计学意义(P>0.05);但肾脏损害、心脏损害的百分率比较,差异有统计学意义(P<0.05)。106例HSP患儿中,根据咽试子细菌培养结果显示链球菌感染18例(16.98%),其中17例为A群化脓链球菌,1例为星座链球菌。A、B两组间皮疹、关节痛、腹痛和(或)便血、便秘的百分率差异无统计学意义(P>0.05);但肾脏损害、心脏损害的百分率差异有统计学意义(P<0.05)。结论儿童过敏性紫癜合并链球菌感染更易出现肾脏、心脏损害,应早期重视,避免重症病例的发生。
Objective To study the infection of streptococcal infection in children with allergic purpura (HSP) and to elucidate the clinical features of children with allergic purpura and streptococcal infection. Methods 106 children with HSP admitted in Hangzhou Children's Hospital from January 2016 to December 2017 were selected. According to the bacterial culture results of throat swabs in 106 children, children with HSP complicated with streptococcal infection were included group A, and the rest were group B. The differences in the percentage of rash, joint pain, abdominal pain and/or blood in the stool, constipation, kidney damage and heart damage were compared between the two groups. Results Among 106 children with HSP, according to the bacterial culture results of the pharyngeal test,18 cases (16.98%) were streptococcal infection, of which 17 cases were group A septicococcus and 1 case was streptococcus. The difference of the percentage of rash, joint pain, abdominal pain and/or blood in the stool and constipation between the two groups was not significantly different(P>0.05). However, However, the difference in the percentage of kidney damage and heart damage was statistically significant (P<0.05). Conclusion Children with allergic purpura combined with streptococcal infection are more likely to have kidney and heart damage. Early attention should be paid to avoid the occurrence of severe cases.
