摘要
目的探讨甲壳质酶蛋白40(YKL-40)在小儿支气管哮喘外周血中的表达及其与肺功能的相关性。方法回顾性分析2017年4月至2018年6月本院收治的80例支气管哮喘患儿临床资料,根据ATC评分将所有患儿分为A组(未控制组)与B组(基本控制组),各40例,另选取40例健康儿童为对照组。测定三组受检者血清YKL-40水平及FEV1%、FEV1/FVC水平,并分析血清YKL-40与FEV1%、FEV1/FVC相关性。结果A、B组血清YKL-40水平分别为(165.32±13.77)ng/ml、(122.61±14.94)ng/ml,均高于对照组(109.33±6.29)ng/ml,且A组与B组FEV1%、FEV1/FVC分别为(50.08±3.09)%、(56.35±3.36)与(79.97±3.55)%、(65.48±4.26),均低于对照组的(92.57±4.97)%、(75.50±4.53),差异均有统计学意义(均P<0.05);经Pearson相关性分析,血清YKL-40水平与肺功能指标中FEV1%及FEV1/FVC均呈负相关性(r<0,P<0.05)。结论支气管哮喘患儿的YKL-40水平在外周血中呈明显上升趋势,且随着肺功能指标水平的增加而降低,故可作为判断哮喘病情发展程度的一项重要血清学指标。
Objective To investigate the expression of chitin protein 40 (YKL-40) in the peripheral blood of children with bronchial asthma and its correlation with pulmonary function. Methods The clinical data of 80 children with bronchial asthma admitted into our hospital from April, 2017 to June, 2018 were retrospectively analyzed. According to the ATC score, all the children were divided into group A (40 cases) and group B (40 cases). Another 40 healthy children were selected as a control group. The serum YKL-40, FEV1%, and FEV1/FVC in the three groups were measured. The correlation of YKL-40 with FEV1% and FEV1/FVC was analyzed. Results The serum YKL-40 level was higher and the FEV1% and FEV1/FVC were lower in group A and group B than in the control group [(165.32±13.77) ng/ml vs.(122.61±14.94) ng/ml vs.(109.33±6.29) ng/ml,(50.08±3.09)% vs.(79.97±3.55)% vs.(92.57±4.97)%, and (56.35±3.36) vs.(65.48±4.26) vs.(75.50±4.53);all P < 0. 05)]. Pearson correlation analysis showed that the serum YKL-40 level negatively correlated with FEV1% and FEV1/FVC (r <0, P < 0.05). Conclusions The level of YKL-40 in the peripheral blood of children with bronchial asthma increases obviously, and decreases with the increase of pulmonary function index, so it can be used as an important serological index to judge the degree of asthma development.
