摘要
目的探讨补肺活血胶囊联合辛伐他汀治疗慢性阻塞性肺疾病(COPD)并肺动脉高压(PH)的临床疗效及安全性。方法选择医院2015年1月至2017年12月收治的COPD并PH患者93例,按随机数字表法分为对照组(46例)和治疗组(47例)。两组患者均给予辛伐他汀治疗,治疗组患者联用补肺活血胶囊,两组均连续治疗3个月。结果治疗组总有效率为82.98%,明显优于对照组的65.22%(χ~2=4.700,P=0.030);治疗后,两组用力肺活量(FVC)、第1秒用力呼气容积(FEV_1)、凝血酶时间(TT)、活化部分凝血酶原时间(APTT)、凝血酶原时间(PT)、动脉血氧分压(PaO_2)、一氧化氮(NO)及一氧化氮合酶(NOS)均明显高于治疗前,肺动脉收缩压(PASP)、脑钠肽(BNP)、纤维蛋白原(FIB)、动脉血二氧化碳分压(PaCO_2)均低于治疗前,且治疗组变化均优于对照组(P <0.05)。两组治疗过程中均无严重不良反应发生。结论补肺活血胶囊联合辛伐他汀治疗COPD并PH的临床疗效显著,能明显降低肺动脉收缩压,改善心肺功能、血气水平及血液高凝状态,升高血清NO和NOS水平,且安全性较好,值得临床推广应用。
Objective To investigate the clinical effect and safety of Bufei Huoxue Capsules combined with simvastatin in the treatment of chronic obstructive pulmonary disease(COPD) complicated with pulmonary hypertension(PH).Methods Totally 93 patients with COPD complicated with PH admitted to our hospital from January 2015 to December 2017 were selected and divided into the control group(46 cases) and the treatment group(47 cases) according to the random number table method.The patients in the two groups were treated with simvastatin,on this basis,the patients in the treatment group were treated with Bufei Huoxue Capsules.The two groups were treated continuously for 3 months.Results The total effective rate of the treatment group was 82.98%,which was significantly better than 65.22% of the control group(χ^2= 4.700, P = 0.030).After treatment,the forced vital capacity(FVC),forced expiratory volume in one second(FEV1),thrombin time(TT),activated partial thrombin time(APTT),prothrombin time(PT),arterial partial pressure of oxygen(PaO2),nitric oxide(NO) and nitric oxide synthase(NOS) in the two groups were significantly higher than those before treatment, while the pulmonary arterial systolic pressure(PASP),brain natriuretic peptide(BNP),fibrinogen(FIB),partial pressure of carbon dioxide(PaCO2) in arterial blood were lower than those before treatment,the change in the treatment group was better than that in the control group(P < 0.05).There was no serious adverse reaction during the treatment in the two groups.Conclusion Bufei Huoxue Capsules combined with simvastatin in the treatment of COPD complicated with PH have significant clinical effect, they can significantly reduce PASP, improve cardiopulmonary function, blood gas level and blood hypercoagulability, and increase the levels of serum NO and NOS, which are safe and worthy of clinical promotion.
