摘要
目的探讨基于磁共振动态增强扫描获得的血流动力学定量参数直方图在乳腺肿瘤鉴别诊断中的应用价值。方法回顾性分析2018-07~2018-10于我院行常规磁共振及动态对比增强磁共振检查的52例乳腺肿瘤患者,参照病理结果将研究对象分为恶性组及良性组。于后处理软件上绘制全域肿瘤并获得感兴趣区内动态增强扫描定量参数容积转运常数(Ktrans)、血浆速率常数(Kep),血浆容积分数(Vp)的直方图参数,包括平均值、中位数及各百分位数值(10%,25%,50%,75%,90%),分别比较良、恶性组间各参数值的差异,其中差异有统计学意义的参数计算其诊断效能。结果恶性组Ktrans中位数、平均值及各百分位数均大于良性组(P<0.05)。Kep75%、Kep90%及Vp平均值、Vp75%、Vp90%大于良性组,差异有统计学意义(P<0.05)。Ktrans各直方图参数诊断良恶性的受试者工作特征曲线下面积(AUC)较高,高于Kep及Vp各参数。所有参数中,Ktrans平均值诊断效能最大。Ktrans平均值的临界值为0. 982时,敏感度为100%,特异度为88. 9%。结论动态对比增强磁共振(DCE-MRI)定量参数全域直方图分析法在乳腺肿瘤的鉴别诊断中具有重要临床价值。
Objective To explore the applied value of histogram of hemodynamic quantitative parameters obtained from DCE-MRI scanning in differential diagnosis of breast tumor.Methods From July to October in 2018,52 patients with breast tumor were analyzed retrospectively.Firstly,all patients underwent the regular MRI and DCE-MRI.The patients were divided into malignant group and benign group according to pathological results.Secondly,the ROIs of whole field of tumors were drawn on the post-processing software and histogram of quantitative parameters(Ktrans,Kep,and Vp)were obtained.The histogram parameters included mean,median and percentiles(10%,25%,50%,75%,90%).Thirdly,the difference of all parameters between malignant group and benign group was compared,and the diagnose efficiencies of statistically significant parameters were calculated.Results The Ktrans(median,mean and percentiles)in malignant group were higher than that in benign group(P<0.05).Meanwhile,Kep 75%,Kep 90%,Vp mean,Vp 75%,and Vp 90%in malignant group were higher than those in benign group(P<0.05).The AUC of Ktrans histogram parameters were higher,and the diagnose efficiency of mean Ktrans was the highest.When the threshold of mean Ktrans was 0.982,the sensitivity and the specificity were 100%and 88.9%,respectively.Conclusion The global histogram analysis of quantitative parameters from DCE-MRI has significant clinical value in differential diagnosis of breast tumors.
