摘要
目的探讨超声检查在女性压力性尿失禁与膀胱脱垂分型中的应用。方法将2016年1月～2017年12月我院接诊的50例前盆腔脱垂合并压力性尿失禁患者与50例单纯前盆腔脱垂患者分别纳入A组和B组,并选择同期在我院体检中心进行健康体检的50例女性纳入对照组,三组入选对象均接受会阴彩色多普勒超声检查,比较三组对象静息状态下膀胱尿道后角(PUA)、最大Valsalva状态下PUA、膀胱颈下降值、膀胱最低点下降值、尿道旋转角检测结果以及膀胱脱垂分型结果。结果 A、B两组患者静息状态下PUA、最大Valsalva状态下PUA、膀胱颈下降值、膀胱最低点下降值、尿道旋转角、膀胱尿道膨出检出率以及孤立性膀胱膨出检出率均与对照组有明显差异(均P<0.05);A、B两组患者最大Valsalva状态下PUA、膀胱最低点下降值、尿道旋转角、膀胱尿道膨出检出率以及孤立性膀胱膨出检出率亦有明显差异(均P<0.05)。结论不同膀胱脱垂分型盆底解剖结构改变差异明显,超声检查能够准确判断,其中膀胱尿道膨出与压力性尿失禁的发生有紧密联系。
Objective To explore the application of ultrasonography in the classification of female stress incontinence and bladder prolapse.Methods From January 2016 to December 2017,accepts the patients of 50 cases of pre-pelvic prolapse combined with stress urinary incontinence and 50 cases with pure pre-pelvic prolapse in our hospital into group A and group B respectively,and select the same physical examination of 50 health women included in the control group.Three sets of selected objects were accepted perineum color doppler ultrasound examination,and compared the PUA in objects resting state three groups of of,PUA of maximum Valsalva condition,bladder neck decline in value,bladder fall prolapse and urinary bladder,result of urethra rotation Angle test and results of classification of bladder prolapse.Results The PUA,PUA of maximum Valsalva condition,bladder neck decline in value,bulging decreased value of bladder rock bottom,rotation Angle of urethra,detection rate of Cystourethrocele,and detection rate of isolation bulging bladder in A,B groups of patients with resting state were significant differences with the control group (P < 0.05).The PUA of maximum Valsalva condition,decreased value of bladder rock bottom,rotation Angle of urethra,detection rate of cystourethrocele,and detection rate of isolation bulging bladder in A,B groups of patients also exist significant difference ( P <0.05).Conclusion Different types of bladder prolapse have obvious differences in pelvic floor anatomical structure,which can be accurately determined by ultrasonography.Cystourethrocele is closely related to the occurrence of stress incontinence.
