摘要
目的探讨血压负荷对高血压合并冠心病患者冠脉病变严重程度的影响。方法回顾性分析2016年1月~2018年12月在本院就诊的70例高血压合并冠心病患者的临床资料,依据冠脉造影检查结果,将其划分为单支病变组(即单支冠状动脉病变患者,30例)与多支病变组(即多支冠状动脉病变患者,40例),监测上述入选者的24h动态血压,比较两组不同冠脉病变程度血压负荷情况以及血压负荷同冠脉病变支数之间的关系。结果两组白天DBP负荷、白天SBP负荷、夜间DBP负荷、全天DBP负荷、全天SBP负荷对比,无显著差异(P> 0.05);单支病变组夜间SBP负荷低于多支病变组,差异显著(P <0.05);经Logistic回归分析发现,夜间SBP负荷属于高血压合并冠心病患者冠脉病变严重程度的危险因素(OR> 1,P <0.05)。结论血压水平与高血压合并冠心病患者冠脉病变严重程度存在密切关联,夜间SBP负荷为相关独立危险因素,临床应实时观察患者血压变化幅度,并给予有效干预措施。
Objective To investigate the effect of blood pressure load on the severity of coronary artery disease in patients with hypertension and coronary heart disease. Methods The clinical data of 70 patients with hypertension complicated with coronary heart disease who were admitted into our hospital from January 2016 to December 2018 were retrospectively analyzed. According to the results of coronary angiography, the patients were divided into single-vessel lesion group (i.e. single-vessel coronary lesion group, 30 patients) and multi-vessel lesion group (i.e. multi-vessel coronary lesion group, 40 patients). The 24h ambulatory blood pressure of the above selected patients was monitored, and the blood pressure load of the two groups with different coronary lesion degrees and the relationship between blood pressure load and the number of coronary lesion branches were compared. Results There was no significant difference between the two groups in daytime DBP load, daytime SBP load, nighttime DBP load, all-day DBP load and all-day SBP load (P > 0.05). The nighttime SBP load in single lesion group was lower than that in multi-lesion group, with significant difference (P < 0.05);Logistic regression analysis showed that nighttime SBP load was a risk factor for severity of coronary artery disease in patients with hypertension and coronary heart disease (OR > 1, P < 0.05). Conclusion Blood pressure level is closely related to the severity of coronary artery disease in patients with hypertension complicated with coronary heart disease. Night SBP load is an independent risk factor. In clinical practice, the blood pressure change range of patients should be observed in real time and effective intervention measures should be given.
