摘要
目的探讨子宫内膜息肉患者行宫腔镜下电切术与刮匙定点刮除术治疗的临床疗效对比。方法选取与2017年6月至2018年3月本院诊断并收治符合子宫内膜息肉的患者200例,同时整理临床及术后随访资料,按不同手术方式分类及随机数字方法的形式分为两组,对照组100例患者均采用刮匙定点刮除术治疗方法,观察组100例患者均采用宫腔镜下电切术治疗方法,对收集和整理的两组患者手术过程所需的手术时间、手术中出血量、术后疼痛情况评分、月经量(术前、术后)、住院所需时间、术后复发率及临床主观疗效等指标进行对比分析。结果实验组术后统计的手术时间、术中出血量、住院时间(11.55±3.22)min,(13.23±1.20)ml(,4.32±1.11)d均低于观察组,差异均有统计学意义(P<0.001),说明宫腔镜下电切术较刮匙定点刮除术治疗子宫内膜息肉更能有效减少手术时间、术中出血量及住院时间;统计实验组患者术后1 d、3d疼痛情况评分(1.5±0.7,2.1±1.0)明显低于对照组,差异均有统计学意义(P<0.001),说明宫腔镜下电切术治疗子宫内膜息肉较刮匙定点刮除术治疗能减轻患者早期疼痛感;统计术后5 d、7 d疼痛情况评分(2.7±1.3,0.8±0.6)较对照组差异无统计学意义,说明宫腔镜下电切术治疗子宫内膜息肉同刮匙定点刮除术治疗在于术后5 d、7 d改善疼痛临床疗效比较差异无统计学意义。统计实验组患者术前、术后月经量(258.10±55.0,26.40±12.00)均低于对照组,差异均有统计学意义(P<0.001)。统计实验组患者术后复发率及主观症状描述均低于对照组,差异均有统计学意义(P<0.05)。结论宫腔镜下行子宫内膜息肉电切术治疗较刮匙定点刮除术治疗能更清楚、直观、安全的达到预期的效果,减少患者痛苦,提高患者生活质量。
Objective To investigate the clinical efficacy of hysteroscopic electroresection and curette curettage in the treatment of endometrial polyps. Methods 200 cases of endometrial polyps diagnosed and treated in our hospital from June 2017 to March 2018 were selected, and the clinical and postoperative follow-up data were collected. The patients were divided into two groups according to different surgical methods and random numerical methods. In the control group, 100 patients were treated by curettage, while in the observation group, 100 patients were treated by hysteroscopic electrotomy. The operation time and operation time of the two groups of patients were collected and arranged. Mean blood loss, postoperative pain score, menstrual volume (preoperative, postoperative, hospitalization time, postoperative recurrence rate and clinical subjective efficacy) were compared and analyzed. Results The postoperative operative time, intraoperative bleeding volume and hospitalization time in the experimental group were significantly lower than those in the observation group (11.55±3.22)min,(13.23±1.20)ml,(4.32±1.11)d. Hysteroscopic electrocution was more effective than curettage and fixed-point curettage in the treatment of endometrial polyps. The amount of intraoperative bleeding and the length of hospitalization were significantly different (P<0.001).The pain score of 1 day after 3 days was significantly lower than that of the control group (1.5 ±0.7, 2.1±1.0), which indicated that hysteroscopic electroresection was more effective than curettage in the treatment of endometrial polyps. The difference was statistically significant (P<0.001), and there was no significant difference between the control group and the control group (P< 0.001), and the score of pain was (2.7±1.3, 0.8±0.6) on the 5th day after operation, and there was no significant difference between the control group and the control group.
