摘要
目的探讨结核抗体、结核杆菌培养联合结核感染T细胞监测对肿瘤患者合并结核感染的指导意义。方法选取2015年3月至2018年3月本院收治的肿瘤患者432例,均给予结核抗体、结核杆菌培养、结核感染T细胞检测,以临床细菌学检查为对照,分析结核抗体、结核杆菌培养、结核感染T细胞及三者联合对肿瘤患者合并结核感染的诊断情况。结果临床细菌学检查显示,肿瘤患者合并结核感染有80例(18.52%);在诊断肿瘤患者合并结核感染的敏感度、特异度、准确度方面,结核抗体分别为87.50%、76.70%、78.70%,结核杆菌培养分别为37.50%、92.61%、82.41%,结核感染T细胞分别为82.50%、88.64%、87.50%,三者联合时分别为97.50%、98.30%、98.15%,三者联合时明显高于三者单独时,差异有统计学意义(P<0.05)。结论肿瘤患者是结核感染的高危人群,结核抗体、结核杆菌培养、结核感染T细胞监测可作为诊断患者合并结核感染的重要方法,三者联合时,对诊断患者合并结核感染具有更佳的指导意义,值得临床作进一步推广。
Objective To discuss the guiding significance of tuberculosis antibody and tubercle bacillus culture combined with tuberculosis infection T cell monitoring in tumour patients complicated with tuberculosis infection.Methods 432 tumour patients were selected from March 2015 to March 2018,and they were given the detection of tuberculosis antibody,tubercle bacillus culture,and tuberculosis infection T cell monitoring.Taking clinical bacteriology examination as controls,the diagnosis of tuberculosis antibody,tubercle bacillus culture,tuberculosis infection T cell monitoring tuberculosis and their combination for tumour patients complicated with tuberculosis infection were analyzed.Results The clinical bacteriology examination showed that 80 patients(18.52%)had tubercular infection.The sensitivity,specificity and accuracy of tuberculosis antibody were 87.50%,76.70%,78.70%,the tubercle bacillus culture were 37.50%,92.61%,82.41%,the tuberculosis infection T cell monitoring were 82.50%,88.64%,87.50%,and the combination of the three were 97.50%,98.30%,98.15%,respectively.The sensitivity,specificity and accuracy of the combination of the three were obviously higher than those of the three alone(P<0.05).Conclusion The tumor patients are the high risk people for tuberculosis infection.Tuberculosis antibody,tuberculosis bacilli culture and tuberculosis infection T cell monitoring can be used as the important method to diagnose the patients with tuberculosis infection,and the combination of the three has the better guiding significance for the diagnosis of patients with tuberculosis infection,which is worth for further clinical promotion.
