摘要
目的:评价家庭医生签约服务模式对妊娠期糖尿病患者妊娠结局的影响。方法:以2015～2017年期间某社区卫生服务中心先后收治的70例妊娠期糖尿病患者为研究对象,随机将70例患者分为观察组和对照组接受治疗,每组35例患者。对照组糖尿病孕妇接受常规的正常治疗,观察组糖尿病孕妇在对照组的治疗基础上接受家庭医生签约服务管理,帮助妊娠期糖尿病患者康复,分析两组糖尿病患者在妊娠结局以及血糖控制的差异,以评价家庭医生签约服务对妊娠期糖尿病患者妊娠结局以及血糖控制的具体影响。结果:在接受治疗前,观察组与对照组相比较,两组在空腹血糖、餐后2h血糖方面的无显著性差异(P>0.05);治疗15d后,两组糖尿病患者空腹血糖、餐后2h血糖均出现降低,但观察组患者降低程度更明显,与对照组相比差异有统计学意义(P<0.05);观察组糖尿病患者妊娠期高血压、羊水过多、早产、产褥感染、产后出血发生率均低于对照组,差异均有统计学意义(P<0.05)。结论:通过家庭医生签约服务模式治疗的妊娠期糖尿病患者具有明显的治疗效果,能够影响到妊娠结局,减少糖尿病孕妇的并发症、围产儿并发症等不良现象的发生。
Objective:To evaluate the effect of family doctor's contract service model on patients with gestational diabetes mellitus.Methods:In this study,70 patients with gestational diabetes mellitus admitted in a community health service center from 2015 to 2017 were randomly selected and divided into observation group and control group for treatment,35 patients in each group.Diabetic gravida in the control group received routine normal treatment.The diabetic gravida in the observation group received signing services from the family doctor on the basis of treatment in the control group to help patients with gestational diabetes mellitus recover.The differences in pregnancy outcomes and glycemic control between the two groups were analyzed to evaluate the specific effects of family doctor signing services on pregnancy outcome and glycemic control in patients with gestational diabetes mellitus.Results:Before treatment,there was no significant difference between the two groups in fasting glucose and 2-hour postprandial blood glucose(P>0.05).After 15 days of treatment,the fasting blood glucose and 2-hour postprandial blood glucose in both groups decreased,but the decrease of the observation group was more obvious,and the difference was statistically significant compared with the control group(P<0.05).The incidence of hypertension,hydramnios,premature delivery,puerperal infection and postpartum hemorrhage in the observation group were all lower than the control group,the difference was statistically significant(P<0.05).Conclusion:Patients with gestational diabetes who are treated by the family doctor contract service have obvious therapeutic effect,which can affect pregnancy outcomes,reduce the complications in diabetic gravida,perinatal complications and occurance of other undesirable phenomena.
