摘要
目的观察Ⅲ级系统产前超声检查在胎儿先天性心脏病中期筛查中的应用效果。方法以2015年1月至2017年12月间,来本院接受孕中期检查的1 895例孕妇为研究对象,先后给予孕妇常规产前超声检查和Ⅲ级系统产前超声检查,以产后超声心动图复检、手术后证实及终止妊娠后胎儿尸检等方式确诊结果为"金标准",将两种检查方式的诊断结果与"金标准"进行对比,计算两种检查方式的临床诊断价值并进行对比。结果经"金标准"确诊,共有34例为胎儿先天性心脏病,先心病检出率为1.79%,常规产前超声检查中,检出先心病26例,检出率为1.37%,与"金标准"相比,误诊1例,漏诊7例,具有较高的一致性;诊断的灵敏度为73.53%,特异度为99.95%,诊断符合率为99.47%,阳性预测值为96.15%,阴性预测值为99.52%;Ⅲ级系统产前超声检查中,检出先心病32例,检出率为1.69%,与"金标准"相比,误诊0例,漏诊2例,具有较高的一致性;诊断的灵敏度为94.12%,特异度为100.00%,诊断符合率为99.89%,阳性预测值为100.00%,阴性预测值为99.89%;对比常规产前超声检查和Ⅲ级系统产前超声检查的诊断价值,Ⅲ级系统产前超声检查的诊断效能更高,尤其在灵敏度、诊断符合率及阴性预测值上,两者差异有统计学意义(P<0.05)。结论相比于常规产前超声检查,应用Ⅲ级系统产前超声检查进行孕中期胎儿先心病的筛查,诊断价值更高,值得推广。
Objective To observe the effect of prenatal ultrasound examination in the third-stage system on the screening of fetal congenital heart disease. Methods From January 2015 to December 2017, 1 895 pregnant women who were admitted to the hospital for the second trimester of pregnancy were enrolled in the study. They were given routine prenatal ultrasound and grade Ⅲ system prenatal ultrasound for postpartum echocardiography. The results of re-examination, post-operative confirmation and post-pregnancy fetal autopsy were confirmed as "gold standard". The diagnosis results of the two examination methods were compared with the "gold standard", and the clinical diagnostic value of the two examination methods was calculated and compared. Results A total of 34 cases of congenital heart disease were diagnosed by the "gold standard". The detection rate of congenital heart disease was 1.79%. Under routine antenatal ultrasound examination, 26 cases of congenital heart disease were detected, the detection rate was 1.37%. Compared with the "gold standard", 1 case was misdiagnosed and 7 cases were missed. The sensitivity of diagnosis was 73.53%, the specificity was 99.95%, the diagnostic coincidence rate was 99.47%, the positive predictive value was 96.15%, and the negative predictive value was 99.52%.Prenatal ultrasound examination of the grade Ⅲ system showed that 32 cases of congenital heart disease were detected, and the detection rate was 1.69%. Compared with the "gold standard", 0 cases were misdiagnosed and 2 cases were missed, which had high consistency. The sensitivity of diagnosis was 94.12%, specificity was 100.00%, diagnostic coincidence rate was 99.89%, positive predictive value was 100.00%, negative predictive value was 99.89%;Compared with the diagnostic value of routine prenatal ultrasound and grade Ⅲ prenatal ultrasound, the diagnostic efficiency of prenatal ultrasound in class Ⅲ system is higher, especially in sensitivity, diagnostic coincidence rate and negative predictive value.
