不同给药干预辅助羊膜移植术治疗翼状胬肉的近远期疗效差异研究 被引量:5

摘要 目的探究不同给药干预辅助羊膜移植术治疗翼状胬肉疗效的影响。方法选取我院2015年5月至2017年5月收治的120例(150只眼)翼状胬肉患者作为研究对象,将患者按照随机数字表法将患者分为A组、B组和C组。A组患者术后采用环孢素A滴眼液治疗,B组患者于术中采用丝裂霉素C处理创面,C组患者术毕时注射贝伐单克隆抗体术后滴注环孢素A进行治疗。比较3组患者3个月治疗效果及术前、术后1个月、术后3个月、术后12个月,泪膜破裂时间(BUT)、基础泪液分泌试验(S I t)水平。比较两组术后并发症情况。结果 C组痊愈率最高(92. 0%),A组次之(78. 0%),B组最低(48. 0%),差异具有统计学意义(P <0. 05)。C组复发率最低(2. 0%),A组次之(8. 0%),B组最高(30. 0%),差异具有统计学意义(P <0. 05)。3组患者术后BUT、S I t相比,患者均出现先降低后升高趋势,术前3组相比,差异无统计学意义(P> 0. 05);术后1个月、术后3个月相比,C组高于A组,A组高于B组(P <0. 05),术后12个月相比,差异无统计学意义(P <0. 05)。3组术后并发症发生率相比,差异无统计学意义(P> 0. 05)。结论环孢素A能够降低翼状胬肉切除术联合羊膜移植术后复发率,效果优于丝裂霉素C,采用贝伐单克隆抗体能够增强环孢素A治疗效果,维持术后泪膜功能稳定,值得在临床中推广应用。 Objective To investigate the effect of different drug delivery interventions on amniotic membrane transplantation for the treatment of pterygium.Methods A total of 120 patients (150 eyes) with pterygium admitted to our hospital from May 2015 to May 2017 were enrolled. The patients were divided into groups A, B and C according to the random number table method. Patients in group A were treated with cyclosporine eye drops after operation, group B patients were treated with mitomycin C during operation, and patients in group C were injected with bevacizumab after surgery. Three groups of patients were compared for 3 months of treatment effect, preoperative, postoperative 1 month, postoperative 3 months, and postoperative 12 months, tear film rupture time (BUT), basic tear secretion test (S I t) levels. Postoperative complications were compared between the two groups. Results The cure rate was the highest in group C (92.0%), the second in group A (78.0%), and the lowest in group B (48.0%). The difference was statistically significant ( P <0.05). The recurrence rate was the lowest in group C (2.0%), the second in group A (8.0%), and the highest in group B (30.0%). The difference was statistically significant ( P <0.05). Compared with the BUT and S I t, the patients in the three groups showed a trend of decreasing first and then increasing. There was no significant difference between the three groups ( P >0.05). At 1 month and 3 months after surgery, group C was higher than group A, and group A was higher than group B ( P <0.05). At 12 months after surgery, the difference was not statistically significant ( P <0.05). There was no significant difference in the incidence of postoperative complications between the three groups ( P >0.05).Conclusions Cyclosporin A can reduce the recurrence rate of pterygium resection combined with amniotic membrane transplantation, and the effect is better than mitomycin C. The use of endogenous monoclonal antibody can enhance the therapeutic effect of cyclosporine A, and maintain the post
