脑脊液和血清降钙素原在脑室外引流置管术后评估颅内感染风险的临床价值 被引量:1

摘要 目的:分析脑脊液和血清降钙素原在脑室外引流置管术后评估颅内感染风险的临床价值。方法:选择2017年2月至2018年10月神经科接诊的110例自发性脑出血患者。患者均接受脑室引流治疗,分别于手术时、术后1 d、术后7 d、术后14 d抽取静脉血2 m L,且通过脑室导管收集脑脊液5 m L。对比不同时间脑脊液降钙素原(PCT)变化,并分析各指标之间的相关性。结果:随着脑室导管留置时间的延长,患者脑脊液PCT和WBC,血清PCT和WBC均呈现明显的升高趋势(P <0. 05)。脑脊液PCT与血清PCT、脑脊液WBC均呈正相关(r=0. 712、0. 615,P <0. 05)。脑脊液PCT与血清WBC无相关性(r=0. 241,P> 0. 05)。结论:脑脊液和血清降钙素原对脑室导管留置时间有重要的指导价值,最大限度发挥脑脊液的引流作用,有助于提升脑室出血疾病的救治效果。 Objective:To analyze the clinical value of cerebrospinal fluid and serum procalcitonin in evaluating the risk of intracranial infection after external drainage.Methods:110 patients with spontaneous intracerebral hemorrhage were selected from February 2017 to October 2018.All patients were treated with ventricular drainage.At the time of operation,1 day after operation,7 days after operation and 14 days after operation,2 mL of venous blood was drawn,and 5 mL of cerebrospinal fluid was collected by ventricular catheter.The changes of procalcitonin (PCT) in cerebrospinal fluid (CSF) at different time were compared,and the correlation between each index was analyzed.Results:With the prolongation of ventricular catheter indwelling time,PCT and WBC,serum PCT and WBC showed a significant increase trend (P<0.05).CSF was positively correlated with serum PCT and cerebrospinal fluid WBC (r=0.712,0.615,P<0.05).There was no correlation between PCT and serum WBC (r=0.241,P>0.05).Conclusion:Cerebrospinal fluid and serum procalcitonin have important guiding value for intracranial pressure monitoring of ventricular catheter indwelling time,maximize the monitoring of intracranial pressure and the drainage of cerebrospinal fluid,which is helpful to improve the treatment of brain traumatic diseases.
