摘要
目的:探讨早期预防性护理对甲状腺全麻手术患者术后体位综合征的影响。方法:选择2016年1月至2018年8月我院收治的甲状腺全麻手术患者100例,按随机数表法分为两组,各50例。对照组采用常规护理模式干预,观察组采用早期预防性护理模式干预。比较两组颈动脉血流动力学指标、体位综合征发生率。结果:术后观察组患者左侧平均血流速度(Vm)为(26.73±3.21)cm/s高于对照组(23.63±3.15)cm/s,右侧Vm(25.47±3.26)cm/s高于对照组(21.62±3.01)cm/s;左侧椎动脉血流量(Fv)为(245.43±9.31)mL/min高于对照组(204.67±8.64)mL/min,右侧Fv(236.85±9.14)mL/min高于对照组(199.85±8.43)mL/min,差异有统计学意义(P<0.05);术后6h,观察组患者头痛、颈椎疼痛、恶心呕吐、腰肌酸痛及切口疼痛的发生率均低于对照组,差异有统计学意义(P<0.05)。结论:对甲状腺全麻手术患者实行早期预防性护理,可有效改善颈椎血流动力学,减少体位综合征的发生,值得临床推广。
Objective:To investigate the effect of early preventive nursing on postoperative posture syndrome in thyroid surgery patients with general anesthesia.Methods:100 thyroid surgery patients with general anesthesia in the hospital from January 2016 to August 2018 were selected,and they were divided into two groups by random number table,with 50 cases in each group.Control group was given routine nursing intervention,while observation group was given early preventive nursing intervention.The indexes of carotid artery hemodynamics and incidence of posture syndrome were compared between the two groups.Results:After surgery,the left mean flow velocity(Vm)of observation group[(26.73±3.21)cm/s]was higher than control group[(23.63±3.15)cm/s],the right Vm of observation group[(25.47±3.26)cm/s]was higher than control group[(21.62±3.01)cm/s];The left vertebral arterial blood flow volume(Fv)of observation group[(245.43±9.31)mL/min]was higher than control group[(204.67±8.64)mL/min],the right Fv of observation group[(236.85±9.14)mL/min]was higher than control group[(199.85±8.43)mL/min](P<0.05);After surgery for 6h,the incidence of headache,cervical vertebra pain,nausea and vomiting,psoas ache,incision pain in observation group was lower than that in control group(P<0.05).Conclusion:Early preventive nursing for thyroid surgery patients with general anesthesia can effectively improve cervical vertebra hemodynamics,reduce the incidence of posture syndrome,which is worthy of clinical promotion.
