摘要
目的探讨胆道手术后胆道出血的临床特点和治疗体会。方法回顾性分析2016年8月-2017年9月我院收治的58例胆道手术后胆道出血患者的临床资料,总结临床特点,随机将所有患者分为两组,各29例,对照组仅给予一般治疗,观察组行介入栓塞治疗,比较两组临床疗效。结果胆道手术后胆道出血集中在术后1 d-2 d,胆道造影检查可见胆管内有血栓,均伴随不同程度黑便(53.45%)、腹痛(39.66%)、发热(31.03%)等症状,有12例(20.69%)有反复出血。两组均有一定疗效,观察组治疗有效率为82.76%,明显高于对照组的62.07%,两组差异显著(P<0.05)。结论根据胆道手术后胆道出血原因、出血量建议选择介入栓塞治疗方式以提升治疗效果,改善患者预后。
Objective To investigate the clinical characteristics and treatment experience of biliary tract hemorrhage after biliary tract operation.Methods The clinical data of 58 patients with biliary tract hemorrhage after biliary tract surgery admitted to our hospital from August 2016 to September 2017 were retrospectively analyzed.The clinical characteristics were summarized.All patients were randomly divided into two groups,29 cases in each group.The control group was given general treatment only,while the observation group was given interventional embolization treatment,and the clinical effects of the two groups were compared.Results Biliary tract hemorrhage after biliary tract operation was concentrated in 1-2 days after operation.Biliary duct thrombosis was found by cholangiography,accompanied by different degrees of black stool(53.45%),abdominal pain(39.66%),fever(31.03%)and other symptoms.Twelve cases(20.69%)had recurrent hemorrhage.The effective rate of the observation group was 82.76%,which was significantly higher than that of the control group(62.07%).The difference between the two groups was significant(P<0.05).Conclusion According to the causes and amount of biliary bleeding after biliary surgery,interventional embolization is recommended to improve the therapeutic effect and prognosis of patients.
