摘要
目的探讨分析脑动脉瘤介入治疗术后腰大池引流及间断腰穿对脑血管痉挛防治的应用效果。方法选取2016年12月至2017年12月接受治疗的60例患者随机分组,主要分为对照组(实施常间断腰穿,n=30),观察组(实施腰大池引流,n=30),比较两组患者治疗期间脑脊液红细胞计数与脑脊液ET-1水平变化、VMCA变化,以及神经功能缺损评分情况。结果观察组患者3、5、7、10、14 d的脑脊液红细胞计数、VMCA变化的情况明显优于对照组,两组对比差异差异具有统计学意义(P<0.05);观察组患者治脑脊液ET-1浓度明显低于对照组,两组数据对比差异具有统计学意义(P<0.05)。结论共腰大池引流在脑动脉瘤介入治疗术后对脑血管痉挛防治效果具有积极的作用,可以有效地防止血管痉挛的发生,促进已发血管痉挛的缓解,该治疗方法值得在临床上推广应用。
Objective To investigate the effect of lumbar cistern drainage and intermittent lumbar puncture on prevention and treatment of cerebral vasospasm after interventional treatment of cerebral aneurysms. Methods 60 patients who received treatment from December 2016 to December 2017 were randomly divided into two groups, which were divided into control group (routine lumbar puncture, n=30) and observation group (n=30). The cerebrospinal fluid red blood cell count and changes in cerebrospinal fluid ET-1 levels, VMCA changes, and neurological deficit scores were compared between the two groups. Results The changes of cerebrospinal fluid red cell count and VMCA in the 3, 5, 7, 10 and 14 days of the observation group were obviously superior to those of the control group, there was a significant difference between the two groups (P< 0.05). The concentration of ET-1 in the cerebrospinal fluid in the observation group was significantly lower than that of the control group, and there was a significant difference between the two groups, there was a significant difference between the two groups (P<0.05). Conclusion The lumbar cistern drainage has a positive effect on the prevention and treatment of cerebral vasospasm after the interventional therapy of cerebral aneurysm. It can effectively prevent the occurrence of vasospasm and promote the remission of hemangiospasm. The treatment method is worthy of clinical application.
