摘要
目的比较研究脊柱结核Bac T/ALERT3D法、改良罗氏培养法等不同技术与病理标本脓液、病灶壁等结核分枝杆菌培养及药敏试验结果,探讨应用价值较高的技术和标本。方法 2016年2月—2018年2月期间,方便选择该院脊柱结核患者的病理标本作为资料,共96例,包含脓液、病灶壁,并进行结核分枝杆菌培养,分别采用Bac T/ALERT3D法、改良罗氏培养法,并针对培养阳性细菌进行药敏试验,分析不同培养方法、不同病理组织标本的阳性率,并进行比较。结果在BacT/ALERT3D法中,病理标本病灶壁培养结果阳性率45.09%显著高于脓液阳性率28.98%(χ2=16.41,P<0.05),在改良罗氏培养法中,病理标本病灶壁培养结果阳性率31.27%显著高于脓液阳性率16.24%(χ2=18.55,P<0.05),即不同病理标本培养结果均存在差异,且均以病灶壁阳性率最高(P<0.05)。结论与改良罗氏培养法相比,Bac T/ALERT3D法检出率更高,利于结核病的早期诊治,其中病灶壁标本培养阳性率更高,应用价值相对更高,值得推广应用。
Objective To compare the results of culture and drug susceptibility test of Mycobacterium tuberculosis in different techniques such as BacT/ALERT3D method and modified Roche culture method for spinal tuberculosis,such as pus and lesion wall,and to explore techniques and specimens with higher application value.Methods From February 2016 to February 2018,pathological specimens of patients with spinal tuberculosis were selected as the data,including 96 cases of pus,lesion wall,and M.tuberculosis culture,using BacT/ALERT3D method.The modified Roche culture method was convenient carried out,and the drug sensitivity test was carried out for the culture-positive bacteria,and the positive rates of different culture methods and different pathological tissue samples were analyzed and compared.Results In the BacT/ALERT3D method,the positive rate of pathological specimens was 45.09%,which was significantly higher than the pus positive rate of 28.98%and(χ^2=11.41 P<0.05).In the modified Roche culture method,the positive rate of pathological specimens was 31.27%,significantly higher than the pus positive rate of 16.24%(χ^2=18.55,P<0.05),that is,the results of different pathological specimens were different,and the positive rate of the lesion wall was the highest(P<0.05).Conclusion Compared with the modified Roche culture method,the detection rate of BacT/ALERT3D method is higher,which is conducive to the early diagnosis and treatment of tuberculosis.The positive rate of the culture of the lesion wall specimen is higher,and the application value is relatively higher,which is worthy of popularization and application.
