摘要
目的系统评价18F-FDGPET/CT对肝转移瘤的诊断价值。方法计算机检索Pubmed、EMbase、TheCochrane Library、WebofScience、中国生物医学文献数据库、万方数字化期刊数据库和中国期刊全文数据库及维普数据库中关于18F-FDGPET/CT诊断肝转移瘤的文献,检索时间截止至2017年12月。由两位评价员按照预定的纳入与排除标准独立筛选文献、提取资料,按照QUADAS-2工具评价纳入研究的偏倚风险后,采用Stata12.0软件进行Meta分析。结果最终纳入15个研究,共1628个患者,1022个结节。Meta分析结果显示:以患者为单位,12篇(1473例患者)PET/CT研究的SEN合并=0.95(95%CI0.92-0.97)SPE合并=0.94(95%CI0.87-0.97)、AUC=0.98(95%CI0.97-0.99),以病灶为单位的6篇(1022例病灶)PET/CT研究的SEN合并=0.82(95%CI0.61-0.93),SPE合并=0.95(95%CI0.89-0.98)、AUC=0.97(95%CI0.95-0.98);亚组分析结果显示:各亚组PET/CT对肝转移瘤诊断的AUC进行比较,差异均无统计学意义(P>0.05)。结论 18F-FDGPET/CT对肝转移瘤具有较高的诊断价值,尤其是对以病人为单位的病例具有更好的诊断敏感度。
Obejctive To evaluate the diagnostic value of PET/CT in the diagnosis of liver metastases by meta-analysis.Methods We searched databases including CNKI,CBM,VIP,WanFang Data,PubMed,EMbase,web of science and The Cochrane Library until April2017 to collect studies about PET/CT in the diagnosis of liver metastases.Two reviewers independently screened literature according to the inclusion and exclusion criteria,extracted data,and assessed the risk of bias of included studies by using the QUADAS-2tool.Then,meta-analysis was performed by using Stata 12.0 software.Results A total of15 studies from nine countries were included,involving 1628 patients and 1022 nodules.The results of meta-analysis showed that,the pooled sensitivity,specificity and area under SROC curve of PET/CT for diagnosing liver metastases were:a)on per-patient based,0.95(95%CI 0.92-0.97),0.94(95%CI 0.87-0.97)0.98(95%CI 0.97-0.99);b)on perlesion based,0.82(95%CI 0.61-0.93),0.95(95%CI 0.89-0.98),0.97(95%CI 0.95-0.98).The results of subgroup analysis showed that no significant differences of AUC were found among all subgroups(P>0.05).Conclusion PET/CT has a good diagnosis value for liver metastases which can be used as an noninvasion tool for suspected patients with liver metastases,especially on per-patient based.
