摘要
目的探讨影响女性压力性尿失禁近远期疗效及性生活质量的相关因素。方法选取2014年3月至2016年3月于海宁市第二人民医院诊治的146例女性尿失禁患者作为研究对象,根据其疗效分为有效组和无效组,分别为84例和62例,出院时自制调查问卷了解可能影响患者近期疗效的因素并进行分析;对84例有效组患者随访1年,根据患者是否完全复原分为完全恢复组和其他组,分别为50例和34例,自制问卷了解可能影响患者远期疗效及性生活质量的因素并进行分析。结果经单因素分析得知,影响压力性尿失禁近期疗效的因素主要为年龄、体重指数、慢性病情况、是否产妇、分娩方式、分娩次数及治疗方式,差异具有统计学意义(P <0. 05);影响压力性尿失禁远期疗效的因素主要是压力性尿失禁严重程度、治疗方式、手术具体方案、手术时间、是否存在近期并发症及出院后心理状态,差异具有统计学意义(P <0. 05);影响压力性尿失禁性生活质量改善的主要因素为文化程度、心理因素(是否伴发抑郁)、配偶的态度和认知及临床近期疗效,差异具有统计学意义(P <0. 05)。经多因素分析得知,分娩次数和治疗方式是影响压力性尿失禁近期疗效的关键因素,差异具有统计学意义(P <0. 05);术式和出院后心理状态则是影响压力性尿失禁远期疗效的关键因素,差异具有统计学意义(P <0. 05);心理因素及临床近期疗效是影响压力性尿失禁性生活质量的关键因素,差异具有统计学意义(P <0. 05)。结论分娩次数、治疗方式、术式和出院后心理状态分别是影响女性压力性尿失禁近远期疗效的关键因素,而心理因素及临床近期疗效是影响压力性尿失禁性生活质量的关键因素,可采取相应的干预措施。
Objective To explore the related factors affecting short-term and long-term efficacy and quality of sexual life of female patients with stress urinary incontinence. Methods 146 female patients with urinary incontinence in Second People’s Hospital in Haining from March 2014 to March 2016 were selected and divided into effective group( 84 cases) and invalid group( 62 cases) according to the curative effect. The self-made questionnaire was used to understand and analyze the factors that may influence the short-term curative effect. The 84 cases in effective group were followed up for a year and according to whether fully recovered,they were divided into recovery group( 50 cases) and unrecovered group( 34 cases). The self-made questionnaire was used to understand and analyze the factors that might influence the long-term curative effect and quality of sexual life of patients. Results The single factor analysis showed that the factors affecting the short-term curative effect of stress urinary incontinence included age,body mass index,chronic disease,maternal status,delivery mode,delivery times and treatment methods( P <0. 05),and the factors affecting the long-term curative effect of stress urinary incontinence mainly included stress urinary incontinence severity,treatment methods,operation scheme,operation time,short-term complications and mental state after hospital discharge( P < 0. 05). The main factors influencing the quality of sexual life of patients included educational level,psychological factors( whether accompanied by depression),spouse’ s attitude,cognition and clinical short-term curative effect( P < 0. 05). The multivariate analysis showed that the number of delivery and treatment methods were the key factors influencing short-term curative effect( P < 0. 05);the operation scheme andmental state after hospital discharge were the key factors affecting long-term curative effect( P < 0. 05);psychological factors and clinical short-term efficacy were the key factors affecting the quality of sexual life o
