摘要
目的探讨宫颈分泌物中胎盘α-1微球蛋白在胎膜早破中的表达及作用。方法选取2017年4月至2018年4月山东省高密市人民医院诊治的胎膜早破孕妇33例作为研究对象。出现胎膜早破体征的孕妇33例,同期在我院进行孕期检查的正常孕妇33例。对三组孕妇均全部使用两种不同的检测方式进行检测, PH(硝嗪试纸)检测及PAMG-1(胎盘α-1微球蛋)检测。观察比较三组孕妇的一般资料、三组孕妇使用两种不同方式进行检测时的阳性率(准确率)、两种检测方式的特异度及敏感度。结果三组孕妇的一般资料对比差异无统计学意义(P>0.05);PAMG-1检测在破水组孕妇中的阳性率(准确率)为97.0%,显著高于PH测定的84.8%,差异具有统计学意义(P<0.05);PAMG-1检测在疑似破水组孕妇中的阳性率(准确率为66.7%,显著高于PH测定的42.4%,差异具有统计学意义(P<0.05);PAMG-1检测在正常组孕妇中阳性率为0.0%,显著低于PH测定的27.3%,差异具有统计学意义(P<0.05);结果提示,PAMG-1检测阳性率(准确率)显著高于对照组,差异具有统计学意义(P<0.05);PAMG-1检测的特异度为96.8%,敏感度为97.5%;PH测定的特异度为90.3%,敏感度为91.7%,PAMG-1检测的特异度及敏感度显著高于PH测定,差异具有统计学意义(P<0.05)。结论对孕妇宫颈分泌物内的胎盘α-1微球蛋白(PAMG-1)进行检测,能够有效诊断孕妇是否出现胎膜早破,具有较高的特异度及敏感度,具有非常高的临床指导及参考意义,值得妇科临床应用及推广。
Objective To investigate the expression and function of placental alpha-1 microglobulin(PAMG-l)in cervical secretion in premature rupture of membranes(PROM).Methods 33 pregnant women with PROM,33 pregnant women with signs of PROM and 33 normal pregnant women who underwent prenatal examination in our hospital from April 2017 to April 2018 were selected.PH detection(nitrazine dipstick)and PAMG-1 detection were performed on all patients of the three groups.The general data and the positive rate(accuracy rate),specificity and sensitivity of the two methods were observed and compared.Results There was no statistically significant difference in the general data among the three groups(P>0.05).The positive rate(accuracy rate)of PAMG-1 detection in pregnant women with PROM was 97.0%,significantly higher than that of PH detection of 84.8%(P<0.05).The positive rate of PAMG-1 detection in pregnant women with signs of PROM(66.7%)was significantly higher than that of PH detection(42.4%)(P<0.05).The positive rate of PAMG-1 in normal pregnant women was 0.0%,significantly lower than the 27.3%determined by PH(P<0.05).The positive rate(accuracy rate)of PAMG-1 detection was significantly higher than that of the control group(P<0.05).The specificity and sensitivity of PAMG-1 detection(96.8%,97.5%)was significantly higher than that of PH(90.3%,91.7%),with statistically significant differences(P<0.05).Conclusions The detection of PAMG-1 in the cervical secretions of preg-nant women can effectively diagnose the occurrence of PROM in pregnant women,with high specificity,sensitivity and clinical guidance and reference significance,which is worthy of clinical application and promotion.
