Effects of intermittent pneumatic compression on transcutaneous oxygen pressure in dorsum of foot in patients with diabetic lower extremity arteriopathy

摘要 目的探讨空气波压力治疗仪(intermittent pneumatic compression,IPC)对糖尿病下肢动脉病变患者足背经皮氧分压(transcutaneous oxygen pressure,TcPO_2)的影响。方法将80例糖尿病下肢动脉病变患者随机分为观察组40例和对照组40例,2组均给予降糖、降压、降脂,改善循环,营养神经等药物治疗,观察组在此基础上加用IPC治疗。评价治疗前和治疗后14d患者足背TcPO_2和踝肱指数(ankle brachial index,ABI)的变化。结果干预前2组足背TcPO_2差异无统计学意义(P>0.05)。干预后观察组TcPO_2较干预前和对照组增高,差异均有统计学意义(P<0.05)。2组ABI干预前后和组间差异均无统计学意义(P>0.05)。结论 IPC治疗可提高糖尿病下肢动脉病变患者足背TcPO_2,有效改善下肢供血和足部的微循环,从而预防糖尿病足的发生。 Objective To investigate the effect of intermittent pneumatic compression(IPC)on transcutaneous oxygen pressure(TcPO 2)in dorsum of foot in patients with diabetic lower extremity arterial disease.Methods Eighty diabetic lower extremity arterial disease patients were randomly divided into experimental group(40 cases)and control group(40 cases)according to random number table.Both groups were given glucose-lowering,blood pressure and blood lipid lowering,improving circulation,nutritional neurologic and other drug therapy,and the experimental group was treated with IPC therapy instrument.The changes of TcPO 2 and ankle brachial index(ABI)in dorsum of foot before and 14 days after treatment were evaluated.Results There was no statistically significant difference between the two groups of patients with TcPO 2(P >0.05)before intervention.After intervention,TcPO 2 of the experimental group was higher than the control group,and the difference was statistically significant(P<0.05).There was no significant difference between the two groups with ABI before and after intervention(P >0.05).Conclusion The treatment of IPC can improve TcPO 2 of dorsum of foot in patients with diabetic lower extremity arteriopathy,effectively improve blood supply of lower extremity and microcirculation of foot,and prevent the occurrence of diabetic foot.
