摘要
目的:探讨胸腔镜食管癌根治术对患者术后肺功能及生活质量的影响。方法:选取88例行食管癌根治术的患者,分为研究组(行胸腔镜手术)与对照组(行开胸手术);监测评估两组患者肺功能指标,以生活质量测定量表(EORTC QLQC30)及欧洲癌症研究与治疗组织食管癌患者补充量表(EORTC QLQ-OES18)评估两组患者术后生活质量。结果:术后2周,两组患者肺功能指标研究组高于对照组(P<0.05)。术前两组患者生活质量评分差异均无统计学意义(P>0.05);术后1、3、6个月,两组患者吞咽困难、食管癌相关疼痛评分差异均无统计学意义(P>0.05);研究组吻合口反流、进食呛咳、食欲减退及症状性维度评分均低于对照组(P<0.05),功能性维度评分、总体生活质量维度评分均高于对照组(P<0.05)。结论:胸腔镜手术对患者肺功能影响小,值得推广应用。
Objective:To explore the effect of thoracoscopic radical esophagectomy on postoperative pulmonary function and quality of life.Methods:Eighty-eight patients with esophageal cancer who underwent radical resection were divided into two groups:study group(thoracoscopic surgery)and control group(thoracotomy),lung function indexes of two groups were monitored and assessed,and quality of life scale(EORTC QLQ-C30)and European cancer research and treatment group supplementary scale(EORTC QLQ-OES18)were used for esophageal cancer patients to evaluate the postoperative quality of life of the two groups.Results:In the second week after operation,and the pulmonary function indexes of the study group were higher than those of the control group(P<0.05).There were no significant differences in the preoperative life quality score,postoperative dysphagia or esophageal cancer-related pain score at 1,3 and 6 months after operation between the two groups(P >0.05).The score of anastomotic reflux,eating coughing,anorexia and symptomatic dimension in the study group were lower than those in the control group at 1,3 and 6 months after operation(P<0.05),the functional dimension score and overall quality of life score of the study group were higher than those of the control group at 1,3 and 6 months after operation(P<0.05).Conclusions:Thoracoscopic surgery is worthy of popularization for esophageal cancer because of little influence on pulmonary function.
