摘要
目的探讨在异位妊娠的保守手术过程中使用腹腔镜联合甲氨喋呤的治疗效果分析。方法选取2016年5月—2017年8月我院收治的异位妊娠的患者81例作为研究对象,将随机分为三组。甲组采用腹腔镜手术联合甲氨喋呤治疗,乙组采用单独腹腔镜手术治疗,丙组采用传统开腹手术联合甲氨喋呤治疗,分析三组患者的治疗效果、手术时间、术中失血量、术后排气时间、住院时间、月经恢复正常时间、HCG恢复正常时间以及再次妊娠情况。结果比较三组的治疗总有效率,甲组患者的治疗总有效率优于乙组与丙组,差异均具有统计学意义(P<0.05);甲组患者的手术时间、术中失血量与乙组比较,差异无统计学意义(P> 0.05),且甲组患者的手术时间、术中失血量均优于丙组,甲组术后排气时间以及住院时间均优于乙组与丙组,差异均具有统计学意义(P <0.05);甲组月经恢复正常时间、HCG恢复正常时间及再次妊娠情况均优于乙组与丙组,差异均具有统计学意义(P <0.05)。结论在异位妊娠的保守手术过程中使用腹腔镜联合甲氨喋呤的效果明显。缩短手术时间、术后排气时间以及住院时间,降低术中失血量甲,缩短月经恢复正常时间、HCG恢复正常时间,改善再次妊娠情况。
Objective To investigate the effect of laparoscopy combined with methotrexate in conservative surgery for ectopic pregnancy.Methods 81 cases of patients with ectopic pregnancy admitted to our hospital from May 2016 to August 2017 were randomly divided into three groups.Group A was treated with laparoscopic surgery combined with methotrexate. Group B received laparoscopic surgery alone.Group C was treated with traditional laparotomy combined with methotrexate.The therapeutic effect, operation time,intraoperative blood loss,postoperative exhaust time, hospitalization time,menstrual recovery time,HCG recovery time and pregnancy status of the three groups were analyzed.Results Comparing the total effective rate of the three groups,the total effective rate of group A was better than that of group B and group C,and the difference was statistically significant (P<0.05).There was no significant difference in operation time and blood loss between group A and group B (P>0.05).The operation time and intraoperative blood loss of group A were better than those of group C, and the exhaust time and hospitalization time of group A were better than those of group B and group C,the difference was statistically significant (P<0.05).The time of menstruation return to normal,the time of HCG return to normal and the situation of second pregnancy in group A were better than those in group B and group C,and the difference was statistically significant (P<0.05).Conclusion Laparoscopic combined methotrexate is effective in the conservative operation of ectopic pregnancy.Shorten operation time, postoperative exhaust time and hospitalization time,reduce intraoperative blood loss,shorten menstrual recovery normal time,HCG recovery normal time,improve the situation of pregnancy again.
