摘要
目的:探讨负压封闭引流技术治疗脓胸的实验研究。方法:选取2016年10月-2017年12月新西兰大白兔20只并建立脓胸动物模型,依据随机数字表法分为负引组和常规组,每组10只。常规组给予常规引流治疗,负引组给予负压封闭引流技术治疗,比较两组的引流效果、生化指标、胸膜康复情况。结果:术后1、4 d,负引组的胸腔积液引流量、碱性成纤维细胞生长因子(bFGF)水平均明显高于常规组(P<0.05);术后1、4 d,负引组的胸液中蛋白质、白细胞数、葡萄糖水平均明显低于常规组,差异均有统计学意义(P<0.05);术后4 d,负引组的脏层胸膜厚度、成纤维细胞数均明显高于常规组,差异均有统计学意义(P<0.05);术后4 d,负引组肺完全复张率明显高于常规组,差异有统计学意义(P<0.05)。结论:负压封闭引流技术治疗可有效改善脓胸的引流效果、生化指标,可有效促进胸膜组织中成纤维细胞增生及粘连愈合,有利于促进肺复张,值得临床做进一步推广。
Objective:To discuss the experimental study of vacuum sealing drainage in the treatment of empyema.Method:20 New Zealand white rabbits were selected from October 2016 to December 2017 to establish a model of empyema,according to the random number table method,they were divided into negative drainage group and conventional group,with 10 cases in each group.Routine drainage was given in the conventional group,and negative drainage group was given in the negative pressure closed drainage technology.The drainage effect,biochemical indexes and pleural rehabilitation of the two groups were compared.Result:1 and 4 days after surgery,the levels of basic fibroblast growth factor(bFGF)in the pleural effusion drainage volume in the negative drainage group were significantly higher than those in the conventional group(P<0.05).1 and 4 days after surgery,the number of protein leukocytes and glucose levels in the pleural effusion of the negative drainage group were significantly lower than those of the conventional group,with statistically significant differences(P<0.05).4 days after surgery,the number of fibroblasts in the visceral pleural thickness in the negative induction group was significantly higher than that in the conventional group,with statistically significant differences(P<0.05).4 days after surgery,the complete lung retraction rate in the negative citation group was significantly higher than that in the conventional group,with statistically significant difference(P<0.05).Conclusion:The negative pressure closed drainage technique can effectively improve the drainage effect and biochemical index of empyema,and can effectively promote fibroblast proliferation and adhesion healing in the pleural tissue,which is beneficial to the promotion of pulmonary re extension,it’s worth for further clinical promotion.
