摘要
目的探讨绝经后老年女性骨质疏松与肌肉力量和肌肉含量的相关性。方法对2015年1-7月在温州医科大学附属第二医院进行体检的1 168例60岁以上老年女性进行数据收集,数据包括问卷调查、体格检查、体成分测量和血生化指标;肌力水平采用握力体重比表示;肌肉含量采用四肢肌肉含量(Appendicular skeletal muscle mass,ASM)与身高(Height,Ht)的平方之比(ASM/Ht2)表示。采用Logistic回归分析肌肉力量、肌肉含量与骨质疏松之间的关系。结果绝经后老年女性骨质疏松患病率为61.6%(719/1 168)。与骨密度正常者相比,骨质疏松患者的握力体重比和ASM/Ht2均偏低(P<0.05)。Logistic回归分析显示,在调整相关因素后,握力与骨质疏松呈负相关,握力水平越高,骨质疏松患病风险越低(P=0.023),但ASM/Ht2与骨质疏松患病差异无统计学意义(P=0.316)。结论肌肉力量与绝经后老年女性骨质疏松密切相关。提示医护人员应注重绝经后老年女性骨质疏松的预防和治疗,更应重视肌肉力量的训练。
Objective To study the correlation between osteoporosis and muscle strength and muscle content in postmenopausal elderly women.Methods Data of 1 168 elderly women over 60 years old who underwent physical examination in the second affiliated hospital of Wenzhou medical university from January to July 2015 were collected,including questionnaire survey,physical examination,body composition measurement and blood biochemical indicators.The muscle strength level was expressed by the ratio of grip strength to body weight.The muscle content was expressed by the square ratio of the muscle content of the extremities(ASM)to the height(Ht)(ASM/Ht2).Logistic regression was used to analyze the relationship between muscle strength,muscle content and osteoporosis.Result This study showed that the prevalence of osteoporosis in postmenopausal elderly women was 61.6%(719/1 168).Compared with those with normal bone mineral density,the weight-to-grip ratio and ASM/Ht2 of osteoporosis patients were lower(P<0.05).Logistic regression analysis showed that after adjusting the factors,grip strength was negatively correlated with osteoporosis.The higher grip strength level,the lower risk of osteoporosis(P=0.023),but there was no significant difference between ASM/Ht2 and osteoporosis(P=0.316).Conclusion Muscle strength is closely related to osteoporosis in postmenopausal elderly women.It is suggested that medical staff should pay attention to the prevention and treatment of osteoporosis in postmenopausal elderly women,and more attention should be paid to the training of muscle strength.
