摘要
目的:探讨青年人群高尿酸血症(HUA)的相关危险因素。方法:选取2016年7月至2017年6月在上海市普陀区人民医院体检的3 638名青年。记录其年龄、性别,测量体重、身高、血压等,并测定常规生化指标及空腹胰岛素(Fins)、尿微量白蛋白(MAU)。根据是否患HUA分为正常尿酸(NUA)组(3 071例)和HUA组(567例),比较两组临床特征。分析血尿酸(SUA)水平与性别、体重指数、甘油三酯(TG)、Fins、血肌酐(Scr)、高密度脂蛋白胆固醇(HDL-C)、超敏C反应蛋白(hs-CRP)、MAU水平及胰岛素抵抗指数(HOMRA-IR)的关系。结果:男性与女性HUA患病率分别为19.15%(350/1 828)、11.99%(217/1 810),差异有统计学意义(P=0.002)。HUA组hs-CRP、体重指数、HOMA-IR均高于NUA组(均P<0.01)。低体重、正常体重、超重、肥胖者HUA患病率分别为0.0%、13.5%、22.8%、26.1%,组间比较差异有统计学意义(P=0.003)。校正性别、体重指数、TG、HDL-C后,SUA仍与hs-CRP、Fins、HOMA-IR、Scr、MAU呈正相关关系(均P<0.05)。二分类Logistic回归分析结果显示,SUA与体重指数、HOMA-IR、hs-CRP水平呈独立正相关关系(均P<0.01)。结论:青年HUA通常有较高的体重指数,可能存在着胰岛素抵抗和慢性炎症状态。
Objective:To observe the level of serum uric acid(SUA)in a health check-up youth population and the hyperuricemia(HUA)prevalence,and to investigate the risk factors related to the youth HUA.Methods:A total of 3 638 youth(consisting of 1 828 men and 1 810 women)who got health check-up in our hospital from July 2016 to June 2017 were enrolled,and divided into non-HUA(NUA)group(n=3 071)and HUA group(n=567)according to the SUA level.The relationship between SUA level,gender,triglyceride(TG),fasting insulin(Fins),serum creatinine(Scr),high density lipoprotein cholesterol(HDL-C),hypersensitive C-reactive protein(hs-CRP)and urinary microalbumin(MAU)levels and insulin resistance index(HOMRA-IR)was analyzed.Results:Youth prevalence of HUA was 19.15%in males and 11.99%in females(P=0.002).The hs-CRP level,body mass index and HOMA-IR were significantly higher in the HUA group than those in the NUA group(P<0.01).There was significant difference in the prevalence of HUA among the patients with underweight,normal weight,overweight and obesity(0.0%,13.5%,22.8%and 26.1%,respectively,P=0.003).After adjusting for gender,body mass index,TG and HDL-C,SUA was also correlated with hs-CRP,HOMA-IR,Scr and MAU(P<0.05).Logistic regression analysis showed that body mass index,HOMA-IR and hs-CRP level were independent factors influencing the SUA level(P<0.01).Conclusion:Youth HUAs generally had high body mass index.Insulin resistance and chronic inflammation might exist in youth HUA patients.
