摘要
目的比较射频消融和抗心律失常药物治疗持续性心房纤颤(房颤)的长期疗效。方法入选2010年1月～2016年3月于内蒙古自治区人民医院心内科住院治疗的60例持续性房颤患者,病程3～9月,心脏超声显示左心房前后径小于45 mm,左室射血分数大于50%。根据患者接受治疗的方式进行分组,接受射频消融治疗的患者30例,为射频消融组,给予包括环肺静脉隔离及线消融在内的射频消融治疗。另30例患者行抗心律失常药物治疗,为对照组。治疗后进行随访,平均随访时间为3年,对两组患者在出院后是否能够长期维持窦性心律进行分析。结果射频消融组男性23例,女性7例,年龄范围47～72岁,平均年龄(59.8±7.1)岁;对照组男性20例,女性10例,年龄48～73岁,平均年龄(62.8±6.5)岁。两组患者性别、年龄、左房径、左室射血分数比较,差异无统计学意义(P均>0.05)。持续性房颤患者射频消融治疗的长期有效率为46.7%,药物治疗的有效率是16.7%,组间比较差异有统计学意义(P=0.012)。结论对于本研究人群,持续性房颤患者射频消融治疗维持窦性心律的长期疗效优于抗心律失常药物治疗。
Objective To compare the long-term curative effects of radiofrequency ablation(RFA)and antiarrhythmic drugs(AAD)in treatment of persistent atrial fibrillation(AF).Methods The patients with persistent AF(n=60),with disease course from 3 months to 9 months and left atrial anteroposterior diameter<45 mm and LVEF>50%,were chosen from Department of Cardiology in People's Hospital of Inner Mongolian Autonomous Region from Jan.2010 to Mar.2016.The patients were divided,according to therapeutic methods,into RFA group(n=30)received RFA treatment including circumferential pulmonary vein isolation and linear ablation,and control group(n=30)treated with AAD.All patients were followed up for an average 3 y after treatment.Whether the patients could maintain sinus rhythm for long time was analyzed in 2 groups after discharged from hospital.Results There were 23 male cases and 7 female cases aged from 47 to 72 with an average age=(59.8±7.1)years old in RFA group.There were 20 male cases and 10 female cases aged from 48 to 73 with an average age=(62.8±6.5)years old in control group.The difference in sex,age,left atrial diameter(LAD)and LVEF had no statistical significance between 2 groups(all P>0.05).The long-term effective rate was 46.7%in RFA group and 16.7%in control group(P=0.012).Conclusion The long-term effective rate is higher in patients with persistent AF treated with RFA than that in those treated with AAD.
