摘要
目的探讨经尿道双极等离子电切术(PKRP)和尿道前列腺电切术(TURP)治疗良性前列腺增生(BPH)的的临床应用效果。方法选取60例BPH患者,按照随机数字法分为2组,对照组30例予以TURP治疗,研究组30例予以PKRP治疗。比较2组手术相关指标(术中出血量、手术时间、切除腺体质量、导尿管留置时间、住院时间)、并发症发生情况、尿道功能[最大尿流率(Max)、剩余尿量(Ru)、前列腺症状评分(IPSS)]改善情况和生活质量评分(QOL)。结果 2组患者切除腺体质量比较差异无统计学意义(P>0.05),研究组术中出血量显著少于对照组,手术时间、导尿管留置时间、住院时间均短于对照组(均P<0.05)。研究组总并发症发生率显著低于对照组(10.00%比23.33%,P<0.05)。2组患者干预前Max、RU、IPSS、QOL比较差异无统计学意义(均P>0.05),干预后2组Max、RU、IPSS、QOL均较干预前显著降低,且研究组低于对照组(均P<0.05)。结论 PKRP治疗BPH可显著降低患者损伤,术后恢复快,并发症少,尿道功能改善明显,患者具有较高的生活质量。
Objective To investigate the clinical effects of bipolar transurethral plasmakineticprostatectomy(PKRP)and transurethral resection of prostate(TURP)on benign prostatic hyperplasia(BPH).Methods Sixty patients with BPH were randomly assigned to receive either TURP(control group,n=30)or PKRP(study group,n=30).Surgical indicators,complications,maximum urinary flow rate(Max),residual urine volume(Ru),international prostate symptom score(IPSS)and quality of life(QOL)were compared between the two groups.Results There were no significant differences between the two groups in the weight of gland resected,as well as in Max,RU,IPSS and QOL before treatment(P>0.05).Compared with control group,intraoperative blood loss,operation time,catheter indwelling time and hospital stay significantly reduced in study group(P<0.05).Furthermore,the incidence of complications in study group(10.00%)was lower than that in control group(23.33%)(P<0.05).After intervention,the Max,RU,IPSS and QOL decreased in both groups,and those in study group were remarkably lower than those in control group(P<0.05).Conclusion PKRP treatment can significantly reduce injuryand complications,promote postoperative recovery,and improve urethral function and QOL in patients with BPH.
