摘要
目的分析重症地中海贫血(简称地贫)患者并发血流感染(BSI)的临床资料,为临床有效防治地贫合并BSI提供依据。方法回顾性分析2012年1月至2017年12月我院诊断为重症地贫合并BSI患者的临床资料,从其诱发因素、临床特点、病原菌分布及临床预后等情况进行分析。结果共进行了6 549例次血培养,其中地贫患者占1 001例次(15.28%),血培养阳性共190例,地贫患者占8例(4.21%),8例患者均为重症地贫,且均为铁过载。2例行脾切除手术,2例行祛铁胺治疗。共培养出8株菌株,G+性菌2株(25.00%),G-菌6株(75.00%)。结论铁过载、去铁胺治疗、脾脏切除是重症地贫患者发生BSI的高危因素。病原菌以G-性菌为主,早期诊断并及时予以经验性抗菌治疗的预后较好。
Objective To analyze the clinical data of patients with severe thalassemia complicated with bloodstream infection (BSI),so as to provided the evidence for effective prevention and treatment of thalassemia combined with BSI in clinic.Methods The clinical data of patients with severe thalassemia complicated with BSI diagnosed in our hospital from January 2012 to December 2017 were retrospectively analyzed.The prognostic factors,clinical characteristics,distribution of pathogenic bacteria and clinical prognosis were analyzed.Results A total of 6 549 blood cultures were performed,of which 1 001 cases (15.28%) were suffered from thalassemia,190 cases were positive in blood culture,8 cases (4.21%) suffered from thalassemia,and all the 8 patients were severe thalassemia with iron overload.Splenectomy was performed in 2 cases,and deferoxamine treatment was performed in 2 cases.Eight strains were cultured,2 strains of G+ bacteria (25.00%) and 6 strains of G- bacteria (75.00%).Conclusion Iron overload,deferoxamine treatment and splenectomy are high risk factors for BSI in severe thalassemia patients.G- bacteria is the main pathogens.Early diagnosis and timely empirical antimicrobial treatment will have a good prognosis.
