摘要
目的探讨急性心肌梗死(AMI)患者心脏破裂(CR)与细胞外基质(ECM)蛋白、ECM辅助性分子及生长分化因子-15(GDF-15)的相关性,为AMI患者并发CR的临床诊治以及预防提供参考。方法回顾性分析265例AMI患者的临床资料,根据患者是否并发CR将其分为对照组(AMI患者230例)和观察组(AMI并发CR患者35例),比较两组患者血清GDF-15及相关ECM蛋白水平,并观察两组患者的血清基质金属蛋白酶和炎症因子水平,对AMI患者并发CR与ECM蛋白、ECM辅助性分子及GDF-15的相关性进行分析。结果观察组患者的血清GDF-5、MMP-2、MMP-3、IL-1、IL-6、TNF-α水平及Ⅲ型胶原、Ⅳ型胶原、粘连蛋白IOD值明显高于对照组(P<0.05)。AMI并发CR与血清GDF-5水平及Ⅲ型胶原、Ⅳ型胶原、粘连蛋白均呈现正相关(P<0.05)。结论 AMI并发CR患者ECM蛋白、ECM辅助性分子及GDF-15均明显升高,可以为AMI并发CR的提前预防诊断提供参考。
Objective To investigate the relationship between cardiac rupture and extracellular matrix(ECM)protein,ECM accessory molecules and growth differentiation factor-15(GDF-15)in patients with acute myocardial infarction,it provides reference for clinical diagnosis and treatment of acute myocardial infarction complicated with ruptured heart.Methods The clinical data of 265 acute myocardial infarction patients admitted to our hospital from February 2016 to March 2018 were retrospectively analyzed,patients were divided into control group(230 patients with acute myocardial infarction)and observation group(35 patients with acute myocardial infarction and ruptured heart)according to whether the patient complicated with cardiac rupture.The levels of serum GDF-15 and related ECM proteins were compared between the two groups.The levels of serum MMPs and inflammatory cytokines in the two groups were observed and the correlations between cardiac rupture and ECM protein,ECM accessory molecules and GDF-15 in patients with acute myocardial infarction were analyzed.Results compared with the control group,the levels of serum GDF-5,MMP-2,MMP-3,IL-1,IL-6,TNF-alpha and IOD value of type III collagen,type IV collagen and adhesion protein IOD in the observation group were significantly higher than those in the observation group,and the difference was statistically significant(P<0.05).There was a positive correlation between AMI and CR and serum GDF-5 level,collagen type III,collagen type IV and adhesion protein(P<0.05).Conclusion The ECM protein,ECM accessory molecules and GDF-15 in patients with acute myocardial infarction complicated with ruptured heart were significantly increased,which could provide reference for the early prevention diagnosis of acute myocardial infarction patients complicated with heart rupture.
