摘要
目的探讨妊娠糖尿病孕妇(GDM)糖化血红蛋白(HbA1c)与新生儿血糖的相关性。方法选取在我院定期产检并分娩的GDM孕妇300例和同期正常孕妇300例为研究对象,检测两组孕妇及其血糖和胰岛素水平的差异,记录两组孕妇剖宫产率、巨大儿及新生儿低血糖发生率,采用Pesrson相关性分析GDM孕妇HbA1c水平与新生儿出生后即时血糖的相关性。结果两组孕妇的空腹血糖以及胰岛素水平比较差异具有统计学意义(P <0. 05)。两组孕妇新生儿出生后即时血糖水平比较差异具有统计学意义(P <0. 05)。正常HbA1c组孕妇剖宫产率、巨大儿发生率以及新生儿低血糖发生率明显低于高HbA1c水平GDM组,比较差异显著(P <0. 05)。GDM孕妇的HbA1c水平与新生儿出生后即时血糖水平呈负相关(r=-0. 86,P <0. 01)。结论 GDM孕妇HbA1c水平直接决定了新生儿脐血中血糖,与妊娠结局密切相关,应及时对孕妇HbA1c水平进行科学监测,GDM及早干预,最大程度减少孕妇血糖紊乱对新生儿的影响。
Objective To investigate the relationship between glycosylated hemoglobin(HbA1c) and blood glucose in neonates with gestational diabetes mellitus(GDM).Methods 300 cases of GDM pregnant women and 300 normal pregnant women were selected as the research subjects.The difference between the two groups of pregnant women and their blood glucose and insulin levels was measured,and the incidence of cesarean section,macrosomia and neonatal hypoglycemia in the two groups was recorded.Blood glucose levels,insulin levels,cesarean section rate,incidence of macrosomia and neonatal hypoglycemia were recorded in the two groups of pregnant women.Pesson correlation was used to analyze the correlation between HbA1c levels in GDM pregnant women and immediate glucose level.Results There were significant differences in fasting blood glucose and insulin levels between the two groups(P<0.05).There was a significant difference in the blood glucose level between the two groups of pregnant women after birth(P<0.05).The cesarean section rate,the incidence of macrosomia and the incidence of neonatal hypoglycemia in the normal HbA1c group were significantly lower than those in the high HbA1c level(P<0.05).The level of HbA1c in pregnant women with GDM was negatively correlated with the blood glucose level after giving birth(R=-0.86,P<0.01).Conclusion The level of HbA1c in pregnant women with GDM directly determines the blood glucose and HbA1c levels in neonatal cord blood,which is closely related to the pregnancy outcome.The level of HbA1c in pregnant women should be monitored scientifically.Early intervention in GDM pregnant women could minimize the impact of maternal blood glucose disorder on neonates.
