摘要
目的探讨液体稠度定量联合果绿染色在气管切开吞咽障碍患者的应用效果。方法选取脑卒中后气管切开伴吞咽障碍患者90例,按照入院时间顺序分为对照组40例和干预组50例。对照组给予洼田饮水试验评估吞咽功能并进行相应进食指导;干预组采用液体稠度定量联合果绿染色进行吞咽功能评估筛查,并通过改变食物性状及液体稠度进行安全进食干预。比较两组进食并发症发生情况及胃管、气管切开套管留置时间。结果干预组吞咽障碍检出例数多于对照组;干预后干预组呛咳、发音异常发生率显著低于对照组(均P <0. 01),胃管、气管切开套管留置时间显著短于对照组(均P <0. 01)。结论液体稠度定量联合果绿染色能够较准确评估气管切开患者吞咽功能,定量改变食物性状及液体稠度进食干预能保证患者摄食安全。
Objective To investigate the effect of applying liquid consistency quantification combined with bright green staining to tracheotomized dysphagia patients.Methods A total of 90 patients tracheotomized after stroke and later developing dysphagia were divided into a control group (40 cases) and an intervention group (50 cases).The former was given dietary instruction after water swallow test,while the latter was given safe-eating intervention (including changing the consistency of foods and fluids) after being assessed with liquid consistency quantification combined with bright green staining.The incidence rates of eating complications and the indwelling time of gastric tube and tracheotomy cannula were compared between the two groups.Results More cases of dysphagia were detected in the intervention group than in the control group; the former also had significantly lower incidence rates of coughing and abnormal pronunciation (P<0.01 for both),and significantly shorter indwelling time of gastric tube and tracheotomy cannula (P<0.01 for both).Conclusion Liquid consistency quantification combined with bright green staining can accurately evaluate the swallowing function of tracheotomized patients.Changing the consistency of foods and fluids could ensure the safety of food intake.
