摘要
目的腹腔镜下修补术与开腹手术对胃十二指肠球部溃疡穿孔患者血小板参数和凝血功能指标的影响。方法对本院收治的64例胃十二指肠球部溃疡穿孔患者展开研究,根据随机数字表达法将其分为观察组与对照组,各32例,观察组患者实行腹腔镜下修补术,对照组患者采取传统开腹手术。探究两组患者的围手术期、血小板参数及凝血功能指标的变化情况。结果观察组患者的术中出血量、拔管及术后肛门排气时间、进食及住院时间均明显少于对照组,差异有统计学意义(P<0.05);观察组患者在PT、APTT上比对照组有所增高,在PLT上有所降低,但差异无统计学意义;两组患者在凝血指标(TT、FIB)和血小板参数(MPV、PDW)等比较,差异均无统计学意义。结论采取腹腔镜下修补术治疗胃十二指肠球部溃疡穿孔患者,相比开腹手术,术中出血量少,创伤小,住院时间短,安全性高,对血小板及凝血功能的影响小,能促进患者康复时间。
Objective To investigate the effects of laparoscopic repair and open surgery on platelet parameters and laparotomy on platelet parameters and coagulation function in patients with perforation of gastroduodenal ulcer.Methods 64 cases with perforation of gastroduodenal ulcer in our hospital were studied.They were divided into observation group and control group according to random number expression method,32 cases in each group.The patients in the observation group were treated with laparoscopic repair,while the control group were received traditional open surgery.The changes of perioperative parameters,platelet parameters and coagulation function indexes of the two groups were investigated.Results The bleeding volume,extubation time,anal exhaust time,food intake and hospitalization time of the patients in the observation group were significantly less than those in the control group(P<0.05);the PT and APTT of the patients in the observation group were higher than those in the control group,and the PLT of the patients in the observation group was lower than those in the control group,but there was no significant difference between the two groups in coagulation.There was no significant difference in blood index(TT,FIB)and platelet parameters(MPV,PDW).Conclusion Compared with open surgery,laparoscopic repair for perforated gastroduodenal ulcer has less bleeding,less trauma,shorter hospitalization time,higher safety,less impact on platelet and coagulation function,and can promote the recovery time of patients.
