不同屏气方式对CT冠脉成像图像质量的影响 被引量:1

Effects of Different Hold Breathing Methods on Quality of Spiral CT Coronary Artery Angiography

摘要 目的:通过研究不同屏气法对受检者心率稳定情况及CT冠状动脉造影图像质量的影响,旨在提高冠状动脉造影检查技术水平。方法:选取100例于本院行64排CT冠脉成像检查的受检者为研究对象,按随机数字表法分为两组:A组(吸气后屏气组)和B组(呼气后屏气组),每组50例。对两组受检者进行不同的屏气训练,检查时分别记录两组受检者屏气后心率暂时稳定时间、心率波动情况,评估两组冠状动脉节段的图像质量。结果:A组受检者达到心率稳定的平均时间为(3.96±1.01)s,B组为(3.44±0.93)s,两组比较差异有统计学意义(P<0.05);A组中54%(27/50)的受检者心率波动较缓,B组有64%(37/50),两组比较差异有统计学意义(P<0.05)。共获取1 300个冠状动脉节段图像,其中A组74.8%(486/650)的冠脉节段图像为合格图像,B组79.5%(517/650),两组受检者冠脉成像图像质量比较差异有统计学意义(P<0.05)。结论:受检者呼气后屏气能在更短的时间内达到暂时稳定,心率波动显著更缓且冠脉成像质量更佳,故呼气后屏气的方式更有利于提高CT冠脉成像图像质量。 Objective:To study the effect of different hold breathing methods on the patient’s heart rate stability and the quality of the 64-row spiral CT coronary artery angiography,which was designed to improve the technique level of the 64-row spiral CT coronary artery angiography.Method:A total of 100 subjects who underwent 64-slice CT coronary angiography in our hospital were selected as subjects.They were randomly divided into two groups according to the random number table,group A(breathing after inhaling group) and group B(breathing after exhaling group),50 cases in each group.Different breath-holding trainings were performed on two groups of subjects.The time of heartbeat stability and heart rate fluctuations of two groups of subjects after breath holding were recorded,and the image quality of the coronary artery segments of two groups were evaluated. Result:The average time to achieve heart rate stability in group A was(3.96±1.01)s,and in group B was(3.44±0.93)s, the difference between two groups was statistically significant(P<0.05).In group A,54%(27/50) of the subjects had a slower heart rate fluctuation,and group B had 64%(37/50),the difference between two groups was statistically significant(P<0.05).A total of 1 300 coronary artery segment images were obtained,74.8%(486/650) of the coronary segment images in group A were eligible images,and group B had 79.5%(517/650),there was a statistically significant difference in the quality of coronary imaging between two groups(P<0.05).Conclusion:After the subject exhales,the breath holding can be temporarily stabilized in a shorter period of time,the heart rate fluctuation is significantly slower and the coronary imaging quality is better,so the manner of holding the breath after exhalation is more conducive to improving the image quality of CT coronary imaging.
