摘要
目的:观察早期综合干预对失禁性皮炎(incontinence-associated dermatitis,IAD)的应用效果。方法:采取便利取样法将243例失禁患者随机分为对照组(n=121)和观察组(n=122),对照组给予常规治疗干预,观察组采用早期综合干预,比较两组患者失禁相关性皮炎发生率、发生时间及满意度情况。结果:干预后,观察组IAD发生率为18. 85%,对照组为37. 19%,两组比较差异有统计学意义(P <0. 05);观察组IAD形成时间为3～10 d,中位数6 d,对照组IAD形成时间1～5 d,中位数3 d,差异具有统计学意义(P <0. 05);观察组患者治疗皮炎时间(3. 2±0. 3) d,比对照组治疗周期短近2 d,差异具有统计学意义(t=19. 8,P <0. 05);观察组患者发生IAD前的平均护理时间(83. 21±45. 30) min/d,未发生IAD患者的平均护理时间(80. 70±40. 13) min/d,对照组患者发生IAD前的平均护理时间(64. 23±40. 84) min/d,未发生IAD患者的平均护理时间(61. 59±38. 33) min/d,差异具有统计学意义(P <0. 05);观察组患者的总体满意率是86. 07%,显著超过对照组的65. 29%(P <0. 05)。结论:对失禁患者早期进行综合干预,可使得患者失禁相关性皮炎的发生率降低,推迟其发生时间,提高患者对护理的满意度。
Objective:To analyze the effect of the early comprehensive nursing on incontinence-associated dermatitis(IAD).Methods:243 patients with incontinence were randomly divided into the control group(n=122)and the observation group(n=124)by convenience sampling,the patients in the control group were given routine nursing care,the observation group was given the early comprehensive nursing.The Incidence,occurrence time and satisfaction of incontinence associated dermatitis were compared between the two groups.Results:The incidence of IAD in the observation group patients was 17.5%,the incidence of IAD in the control group patients was 32.5%,the difference between the two groups was statistically significant(P<0.05).The IAD formation time in the observation group was 3~10 d,the median was 6 d,the IAD formation time in the control group was 1~5 d,the median was 3 d,the difference between the two groups was statistically significant(P<0.05).The treatment time of dermatitis in the observation group was(3.2±0.3)d,which was nearly 2 days shorter than that in the control group,the difference between the two groups was statistically significan(t=19.8,P<0.05).The average nursing time of patients in observation group before IAD was(83.21±45.30)min/d,that of patients without IAD was(80.70±40.13)min/d,the average nursing time of patients in control group was(64.23±40.84)min/d,and that of patients without IAD was(61.59±38.33)min/d.The difference was statistically significant(P<0.05).The overall satisfaction rate of the observation group was 86.07%,which was significantly higher than 65.29%of the control group(P<0.05).Conclusion:Early comprehensive intervention for incontinence patients can reduce the incidence of incontinence-related dermatitis,delay its occurrence and improve patient′s satisfaction with nursing.
