摘要
目的:探讨甲状腺乳头状癌常规超声和超声造影声像图特征及其与病理组织学变化之间的相关性。方法:回顾性分析196例甲状腺疾病患者共253个甲状腺结节(141个恶性、112个良性)超声造影声像图特征,与病理学检查结果对照,用单因素(χ2检验)和多因素(Logistic回归)分析甲状腺结节常规超声和超声造影声像图特征及其与病理组织学变化的关系。结果:单因素和多因素分析显示,边缘不规则、微小钙化灶、纵横比≥1、不均匀性低增强、增强后肿块范围缩小和增强后肿块形态不规则与甲状腺癌病理组织变化关系密切(χ~2=8.677、13.074、15.251、1.080、79.441和7.595,OR=3.714、35.442、6.632、22.332、29.296和18.678,P均<0.01),能较准确地判断甲状腺癌。周边环形高增强与甲状腺良性结节密切相关(χ~2=67.143,OR=35.264,P=0.000)。结论:甲状腺乳头状癌超声造影声像图特征与病理组织学变化密切相关,超声造影对提高超声诊断甲状腺癌的准确率具有重要意义。
Objective:To explore the correlation between contrast-enhanced ultrasonographic features and pathological changes in papillary thyroid carcinoma.Methods:The contrast-enhanced ultrasonographic features of 196 thyroid patients with 253 lesions(141 malignant,112 benign)and the pathological changes were retrospectively analyzed.The ultrasonographic results were compared with pathological results by univariate(χ^2 test)and multivariate analysis(Logistic regression).Results:Irregular tumor shape,microcalcification,anteroposterior to transverse diameter ratio(A/T)≥1,inhomogeneously low enhancement,narrow range and enhanced irregular tumor shape were correlated well with pathological changes(χ^2=8.677,13.074,15.251,1.080,79.441 and 7.595,OR=3.714,35.442,6.632,22.332,29.296 and 18.678,P<0.01).Conclusion:Contrast-enhanced ultrasonographic features are correlated with pathological changes in papillary thyroid carcinoma.Contrast-enhanced ultrasound could improve detection accuracy of papillary thyroid carcinoma.
