摘要
目的探析颈颅对比增强磁共振血管造影(CE-MRA)使用低剂量对比剂的临床应用价值。方法随机选取该院在2017年1月—2018年1月期间收治的92例疑似颈颅部血管病变患者,将患者分为对照组46例和治疗组46例,其中对照组患者使用1.5 T磁共振扫描仪行颅颈部动脉3D CE-MRA,使用对比剂量为0.30 mmol/kg,治疗组患者实施1.5 T磁共振扫描仪行颅颈部动脉3D CE-MRA,使用对比剂量为0.20 mmol/kg,比较两组患者的静脉污染评分、图像质量评分、颅脑血管分支级别、动脉瘤诊断敏感性。结果治疗组患者静脉污染评分(1.79±0.62)分、图像质量评分(3.28±0.64)分、颅脑血管分支级别(3.19±0.47)分均明显高于对照组(1.61±0.58)分、(3.01±0.70)分、(3.02±0.30)分,差异有统计学意义(t=3.338,6.535,4.021,P<0.05);对照组动脉瘤诊断敏感度75.0%明显低于治疗组100.0%,差异有统计学意义(χ~2=12.260,P<0.05)。结论颈颅CE-MRA使用低剂量对比剂的临床应用价值较高,与0.30 mmol/kg钆对比剂相比,0.20mmol/kg对比剂图像质量良好,值得临床推广应用。
Objective To investigate the clinical value of contrast-enhanced magnetic resonance angiography (CE-MRA) in the use of low-dose contrast agents.Methods A total of 92 patients with suspected cervical cranial vascular disease admitted to the hospital from January 2017 to January 2018 were convenient enrolled.The patients were divided into 46 patients in the control group and 46 patients in the treatment group.The control group used 1.5T magnetic resonance imaging.The scanner was performed with 3D CE-MRA in the cranial and cervical artery,and the contrast dose was 0.30 mmol/kg.The patients in the treatment group underwent a 1.5T magnetic resonance scanner to perform 3D CE-MRA of the cranial and cervical artery,using a contrast dose of 0.20 mmol/kg.The venous contamination score,image quality score,cranial vascular branch level,and diagnostic sensitivity of aneurysm were compared between the two groups.Results In the treatment group,the venous pollution score (1.79±0.62)points,the image quality score (3.28±0.64)points,and the cranial vascular branch level (3.19±0.47)points were significantly higher than the control group (1.61±0.58)points,(3.01±0.70)points,(3.02±0.30)points,the difference was significant (t=3.338,6.535,4.021,P<0.05);the diagnostic sensitivity of the control aneurysm was 75.0% significantly lower than that of the treatment group (100.0%),the difference was significant (χ^2=12.260,P<0.05).Conclusion The clinical value of cervical-cranial CE-MRA with low-dose contrast agent is higher.Compared with 0.30mmol/kg bismuth contrast agent,the image quality of 0.20mmol/kg contrast agent is good,which is worthy of clinical application.
