摘要
目的:探讨阿托伐他汀对脑卒中患者颈动脉弹性、氧化应激及血液流变学的影响。方法:选取2017年10月至2018年7月牡丹江医学院附属红旗医院收治的脑卒中患者920例作为研究对象,以抽签法分为观察组和对照组,每组460例。对照组患者单纯给予常规治疗,观察组患者在对照组的基础上加用阿托伐他汀治疗。比较两组患者的颈动脉弹性指标[双侧颈动脉硬化度(β)、弹性系数(Ep)、顺应性(AC)、脉搏波传导速度(PWVβ)及膨大指数(AI)]、氧化应激指标及血液流变学指标水平的差异。结果:治疗后,观察组患者β、Ep、AC、PWVβ及AI等指标水平明显低于对照组,差异均有统计学意义(P<0.05);观察组患者氧化低密度脂蛋白、丙二醇、超氧化物歧化酶及谷胱甘肽过氧化物酶水平明显优于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者血沉、全血低切黏度、全血高切黏度、血浆黏度及纤维蛋白原水平明显优于对照组,差异均有统计学意义(P<0.05)。结论:阿托伐他汀可有效改善脑卒中患者的颈动脉弹性、氧化应激及血液流变学。
OBJECTIVE:To investigate the effects of atorvastatin on carotid elasticity, oxidative stress and hemorheology in stroke patients. METHODS:920 patients with stroke admitted into Hongqi Hospital Affiliated to Mudanjiang Medical University from Oct. 2017 to Jul. 2018 were extracted to be divided into observation group and control group via the lottery, with 460 cases in each. Patients in the control group were given conventional treatment alone, while the observation group received atorvastatin on the basis of the control group. Carotid artery elasticity indexes [bilateral carotid atherosclerosis(β), elasticity coefficient(Ep), compliance(AC), pulse wave conduction velocity(PWVβ) and swelling index(AI)], oxidative stress indexes and hemorheology related indexes were compared between two groups. RESULTS:After treatment, the levels of β, Ep, AC, PWVβ and AI in observation group were significantly lower than those in control group, and the differences were statistically significant(P<0.05). The levels of oxidized low density lipoprotein, propylene glycol, superoxide dismutase and glutathione peroxidase in observation group were significantly better than those in control group, and the differences were statistically significant(P<0.05). After treatment, the erythrocyte sedimentation rate, whole blood low-cut viscosity, whole blood high-viscosity, plasma viscosity and fibrinogen level in observation group were significantly better than those in control group, and the differences were statistically significant(P<0.05). CONCLUSIONS:Atorvastatin is effective in improving carotid elasticity, oxidative stress and hemorheology in stroke patients.
