摘要
目的研究百草枯中毒后外周血CD4^+T细胞PD-1/PD-L1信号通路的表达及其对T细胞免疫的影响。方法收集2017-06~2018-06我院收治的百草枯中毒急性期患者30例(口服20%百草枯溶液10-30 ml),选取同时期本院健康体检者30例为对照组。以中毒后24,72,120 h为观察点,流式细胞术检测外周血CD4^+T细胞PD-1、PD-L1阳性率,Western blot法检测外周血单个核细胞PD-1、PD-L1蛋白表达,酶联免疫吸附法检测血清TNF-ɑ和IL-10浓度。结果中毒组CD4^+T细胞PD-1阳性率在中毒后24,72 h均高于对照组(均P <0. 01),PD-L1阳性率在各观察点均高于对照组(均P <0. 05);蛋白免疫印迹实验结果显示:外周血单个核细胞PD-1、PD-L1蛋白表达在各观察点较对照组均明显升高(均P <0. 05);与对照组比较,血清TNF-ɑ及IL-10水平在中毒后24 h(P <0. 05)、72 h(P <0. 05)明显升高,但中毒后120 h差异无统计学意义(P>0. 05)。结论百草枯中毒后外周血CD4^+T淋巴细胞PD-1/PD-L1信号通路明显激活,抑制CD4^+T细胞分泌炎性细胞因子,在百草枯中毒后免疫反应中发挥了一定的作用。
Objective To investigate the expression of the PD-1/PD-L1pathway in peripheral blood CD4^+T lymphocyte cells and its effect on T cell immunity after paraquat poisoning.Methods Thirty paraquat poisoning patients(oral paraquat poisoning 10-30ml)admitted into our hospital from June 2017 to June 2018 were enrolled.Another 30 subjects who received physical examination in the same hospital in the same period were enrolled as control group.The PD-1and PD-L1positive rates of CD4^+T cells in peripheral blood were analyzed after paraquat poisoning for 24h,72h and 120h.At the same time,the PD-1and PD-L1protein expression in PBMC and serum levels of TNF-ɑand IL-10were also detected.Results Compared with control group,the PD-1positive rate of CD4^+T cells was significantly increased in paraquat poisoning group at24h and72h after poisoning(both P<0.01),and the PD-L1positive rate was also significantly increased at24h,72h and120h after poisoning(both P<0.05).The protein expression of PD-1and PD-L1in peripheral blood mononuclear cells were significantly higher in paraquat poisoning group than in control group at each observation point.Meanwhile,compared with control group,serum TNF-ɑand IL-10levels increased significantly in paraquat poisoning group at24h and72h after poisoning(P<0.05),but there was no significant difference at 120h(P>0.05).Conclusion The PD-1/PD-L1pathway in peripheral blood CD4^+T lymphocyte is significantly activated after paraquat poisoning.It can inhibit the secretion of inflammatory cytokines by CD4^+T cells and play a certain role in the immune response of paraquat poisoning.
