摘要 目的探究分析HD(血液透析)联合HP(血液灌流)和单纯HD(血液透析)对顽固性高血压的尿毒症患者心脏结构和功能的影响。方法选取本院2016年6月~2017年12月收治的顽固性高血压尿毒症患者84例进行研究,以随机数字表法分为对照组(n=42)和观察组(n=42),对照组单纯采用血液透析治疗,观察组采用血液透析和血液灌流治疗。比较两组患者治疗前后的血压变化情况,同时观察比较患者左房内径(LAD)、左室舒张末内径(LVDd)、左室收缩末内径(LVDs)、左室射血分数(LVEF)等。结果治疗前,两组患者的高血压症状差异无统计学意义(P> 0.05),治疗后,观察组患者的收缩压、舒张压均比对照组低,差异有统计学(P <0.05);观察组患者的左房内径、左室舒张末内径、左室收缩末内径分别低于对照组,观察组患者的左室射血分数显著高于对照组,差异有统计学(P <0.05)。结论在顽固性高血压尿毒症患者临床中采用血液透析和血液灌流联合治疗,能够稳定患者的血压,同时也能改善患者的心脏结构,值得在临床中运用。 Objective To investigate the effects of HD(hemodialysis) combined with HP(blood perfusion) and HD alone(hemodialysis) on cardiac structure and function in uremic patients with refractory hypertension. Methods A total of 84 patients with refractory hypertensive uremia admitted to our hospital from June 2016 to December 2017 were randomly divided into control group(n=42) and observation group(n=42). The control group was treated with hemodialysis alone, and the observation group was treated with hemodialysis and blood perfusion. The changes of blood pressure before and after treatment were compared between the two groups. The left atrial diameter(LAD), left ventricular end diastolic diameter(LVDd), left ventricular end systolic diameter(LVDs), and left ventricular ejection fraction(LVEF) were compared. Results Before treatment, there was no significant difference in the symptoms of hypertension between the two groups(P > 0.05). After treatment, the systolic blood pressure and diastolic blood pressure of the observation group were lower than those of the control group(P < 0.05). The inner diameter, left ventricular end diastolic diameter and left ventricular end systolic diameter were lower than the control group. The left ventricular ejection fraction of the observation group was significantly higher than that of the control group(P < 0.05). Conclusions The combination of hemodialysis and hemoperfusion in patients with refractory hypertensive uremia can stabilize the patient’s blood pressure and improve the patient’s heart structure. It is worthy of clinical application.
