摘要
目的:探讨食管内放射性支架置入对中晚期食管癌患者吞咽困难程度的改善、预后的影响。方法:选取118例中晚期食管癌患者进行回顾性研究,其中57例患者采用放射性支架置入(研究组)、对照组61例采用普通覆膜食管内支架置入治疗,对比两组患者治疗前后的吞咽指数、生存质量、预后情况。结果:治疗后,两组患者的吞咽困难程度较治疗前均显著的缓解,研究组患者的吞咽困难程度Stooler分级较对照组缓解更加明显(P<0.05);术后,研究组患者食管再狭窄率、反流性食管炎发生率均低于对照组(P<0.05);术后3个月,研究组的生存质量评分显著的高于对照组(P<0.05);随访一年,研究组生存率36.84%(21/57)与对照组的26.23%(16/61)比较差异无统计学意义(P>0.05);研究组的中位生存时间10.5个月,对照组8.0个月,研究组的中位生存时间高于对照组(P<0.05)。结论:食管内放射性支架置入对中晚期食管癌患者的吞咽困难缓解效果优于普通覆膜支架,降低术后食管再狭窄率,对延长患者生存时间有一定的帮助。
Objective:To investigate the effect of esophageal stent implantation on the improvement of dysphagia and prognosis in patients with advanced esophageal cancer.Methods:a retrospective study was carried out in118patients with advanced esophageal cancer,57of them were treated with radioactive stent implantation(Study Group)and61cases of control group with common covered esophageal stent implantation.The swallowing index and quality of life before and after treatment were compared between the two groups.Results:after treatment,the degree of dysphagia in the two groups was significantly lower than that before the treatment,and the degree of dysphagia Stooler in the study group was more obvious than that of the control group(P<0.05).The rate of restenosis and reflux esophagitis in the study group were lower than that of the control group(P<0.05)after the operation(P<0.05),and3after the operation.The quality in life of the study group was significantly higher than that of the control group(P<0.05).The survival rate of the study group was36.84%(21/57)and26.23%(16/61)in the control group(P>0.05)for one year.The median survival time of the study group was10.5months and the control group was8months.The median survival time of the study group was higher.In the control group(P<0.05).Conclusion:the effect of esophageal stent implantation on the dysphagia of patients with middle and advanced esophageal cancer is better than that of the common covered stent,reducing the rate of restenosis after the operation,and is helpful to prolong the survival time of the patients.
