摘要
目的总结COPD患者实施自我管理教育干预的效果观察。方法通过建立专科疾病自我管理教育小组,制定规范的疾病自我管理教育计划,出院前后进行疾病相关知识的自我护理知识掌握情况的问卷调查。结果提高患者对疾病相关知识的预防、康复训练方法,出院前后自我护理知识掌握情况比较,差异有统计学意义。(r<0.01或r<0.05)。增强了自我保健意识,调动了患者及家属的积极参与及学习自我管理的主动性,从而降低了患者的再入院率。结论实施专科疾病自我管理教育的干预,规范了患者出院后治疗、护理、康复的一体化的监管机制,完善患者出院后到家庭、社区过渡期护理的转变,为发展过渡期护理夯实了基础。
Objective To summarize the effect of self-management education intervention in COPD patients.Methods Through the establishment of specialized disease self-management education group,the establishment of standardized disease self-management education plan,before and after discharge from hospital for disease knowledge of self-care knowledge of the questionnaire survey.Results The prevention and rehabilitation training methods of disease-related knowledge of patients were improved.There were statistically significant differences in self-care knowledge before and after discharge(r<0.01or r<0.05).Enhanced selfcare awareness,mobilized the active participation of patients and their families and the initiative of learning self-management,thus reducing the readmission rate of patients.Clousion The implementation of education intervention of specialized disease selfmanagement standardized the integrated supervision mechanism of treatment,nursing and rehabilitation of patients after discharge,improved the transition of patients to family and community nursing after discharge,and laid a solid foundation for the development of transitional nursing.
