摘要
目的分析超声内镜与多层螺旋CT对胃癌分期诊断价值。方法方便选择2015年1月—2017年12月在该院接受治疗的胃癌患者45例,均以手术后活检病理证实,分别在术前经由超声内镜、多层螺旋CT检查,分析检查结果与病理结果差异。结果超声内镜与病理T1-4分期总相符率为91.11%,与病理N0-3分期总相符率为66.67%,与病理M0、M1分期相符率为66.67%;螺旋CT与病理T1-4分期总相符率为66.67%,与病理N1-3分期总相符率为77.78%,与病理M0、M1分期总相符率为80.00%;超声内镜与病理T分期相符率显著高于螺旋CT (χ~2=8.072 6,P=0.004 5),但N0-3、M0-1间相符率比较数据差异无统计学意义(P>0.05)。结论超声内镜检查肿瘤对胃粘膜的浸润情况好于螺旋CT,但检查肿瘤转移病灶不如螺旋CT,建议在临床实际工作中联合使用,有助于弥补不同检查的局限性,提高诊断准确性。
Objective To analyze the value of endoscopic ultrasonography and multi-slice spiral CT in diagnosis of gastric cancer staging.Methods45cases of patients with gastric cancer treated in our hospital from January2015to December2017were convenient selected,and the patients were proved by the biopsy pathology after surgery,and the patients were respectively for endoscopic ultrasonography and multi-slice spiral CT,and the difference between the examination results and pathological results were analyzed.Results The total coincidence rate of endoscopic ultrasonography and pathological T1-4staging was91.11%,and the total coincidence rate with pathological N0-3staging was66.67%,and the coincidence rate with the pathological M0,M1staging was66.67%,and the total coincidence rate between the spiral CT and pathological T1-4staging was66.67%,and the total coincidence rate with pathological M0,M1staging was80.00%,and the coincidence rate of endoscopic ultrasonography and pathological T staging was obviously higher than that of spiral CT(χ^2=8.0726,P=0.0045),but the difference in the coincidence rate between the N0-3and M0-1was not statistically significant(P>0.05).Conclusion The infiltration of endoscopic ultrasonography for gastric mucosa is better than that of spiral CT,but the test of metastatic tumors is less than the spiral CT,and it is suggested to be used in the clinical practical work,and it contributes to supplementing the limitations of different examinations and improving the diagnosis accuracy.
