摘要
目的研究硬膜外麻醉镇痛在足月妊娠产妇分娩中的应用效果。方法数字随机法抽取2017年5月—2018年5月期间该院产科接收并分娩的足月妊娠产妇86例,根据其镇痛方式进行分组,其中,行导乐分娩镇痛的43例产妇为对照组,另行硬膜外麻醉镇痛的43例产妇为研究组,对比两组镇痛的效果。结果研究组第一产程(7.14±0.65)h、总产程时间(8.46±0.92)h、产后出血量(470.69±16.69)mL优于对照组,差异有统计学意义(t=-12.559 5、-17.728 9、-13.940 1,P<0.05),研究组T1、T2、T3时段VAS评分(4.95±1.24)分、(3.25±0.64)分、(2.14±0.28)分低于对照组,差异有统计学意义差异无统计学意义(t=-4.871 8、-7.001 0、-7.278 7,P<0.05)、Ramsay镇静评分(2.61±0.34)分、(3.16±0.47)分、(3.67±0.71)分优于对照组,差异有统计学意义(t=6.806 3、6.242 0、3.896 1,P<0.05),研究组新生儿Apgar评分(9.46±1.25)分与对照组对比(t=0.428 2,P>0.05)。结论硬膜外麻醉镇痛在足月妊娠产妇分娩中的应用效果确切,有推广的价值。
Objective To study the application effect of epidural narcotic analgesia in the delivery of full-term pregnant delivery women.Methods86cases of full-term delivery women admitted and treated in our hospital from May2017to May2018were selected and divided into groups according to the analgesic mode with43cases in each,the control group underwent the Doula analgesia,while the research group used the epidural narcotic analgesia,and the analgesia effect was compared between the two groups.Results The first labor process,total labor process time and postpartum hemorrhage in the research group were respectively(7.14±0.65)h,(8.46±0.92)h,(470.69±16.69)mL,which were better than those in the control group,the different was statistically significant(t=-12.5595,-17.7289,-13.9401,P<0.05),and the VAS scores at T1,T2,T3in the research group were respectively(4.95±1.24)points,(3.25±0.64)points,(2.14±0.28)points,which were lower than those in the control group,the different was statistically significant(t=-4.8718,-7.0010,-7.2787,P<0.05),and the Ramsay sedation scores were respectively(2.61±0.34)points,(3.16±0.47)points,(3.67±0.71)points,which were better than those in the control group,the different was statistically significant(t=6.8063,6.2420,3.8961,P<0.05),and the Apgar score of newborns was(9.46±1.25)points,and the difference was not statistically significant(t=0.4282,P>0.05).Conclusion The application effect of epidural narcotic analgesia in the delivery of full-term pregnant delivery women is definite,and it is of promotion value.
