梅州地区260例宫颈上皮内瘤变患者人乳头瘤病毒感染状况分析 被引量:5

摘要 目的探讨梅州地区宫颈上皮内瘤变(Cervical Intraepithelial Neoplasia,CIN)患者人乳头瘤病毒(HPV)感染和亚型分布情况。方法将2015年9月—2018年6月于本院就诊的宫颈病变患者,以液基细胞学联合HPV检测方法进行筛查,并行阴道镜下宫颈活检病理诊断为CIN患者260例,分为CIN 1、CIN2和CIN3组,分析3组HPV感染和亚型分布情况。结果 (1)260例CIN患者HPV感染率73. 08%(190/260),其中CIN1感染率54. 84%(68/124),CIN2感染率88. 64%(78/88),CIN3感染率91. 67%(44/48),3组间HPV感染率比较,差异有统计学意义(2=40. 224,P=0. 000),HPV感染率与CIN的级别呈正相关,HPV感染率与CIN病变程度而增加。(2)HPV感染190例CIN患者中,单一感染137例(72. 11%),多重感染53例(27. 89%)。低度CIN1与高度CIN2/3单一和多重感染比较,差异有统计学意义(2=5. 529,P=0. 019)。(3)HPV基因分型结果,CIN1为主要为HPV-11、HPV-16、HPV-6、HPV-52、HPV-58型感染,CIN2/3主要为HPV-16、HPV-52、HPV-58、HPV-31、HPV-33型感染,高危型HPV感染与CIN患病明显相关,但其相关的HPV高危亚型不同。(4)CIN1 HPV感染在25~29岁和35~39岁出现两个高峰,CIN2/3 HPV感染于45~49岁最高,差异有统计学意义(2=41. 148、38. 071,P=0. 000)。结论梅州地区CIN1 HPV感染高峰在25~29岁和35~39岁,CIN2/3 HPV感染高峰在45~49岁,HPV-16是主要致病亚型,感染高危型HPV可增加CIN2/3的患病机会。 Ojbective To investigate the infection rate and genotyping of human papilloma virus(HPV)among patients with cervical intraepithelial neoplasia(CIN)in Meizhou.Methods Patients with cervical lesions who were treated in our hospital from September2015to June2018were screened by liquid-based cytology combined with HPV.The pathological diagnosis of cervical biopsy under colposcopy was260patients with CIN,divided into CIN1in the CIN2and CIN3groups,the distribution of HPV infection and subtypes in the three groups was analyzed.Results The HPV infection rate of1260patients with CIN was73.08%(190/260),of which CIN1infection rate was54.84%(68/124),CIN2infection rate was88.64%(78/88),and CIN3infection rate was91.67%(44/48).The difference of HPV infection rate between the groups was statistically significant(χ^2=40.224,P=0.000).The HPV infection rate was positively correlated with the level of CIN,and the HPV infection rate was increased with the degree of CIN lesions.Of the190patients with CIN infected with2HPV,137(72.11%)were single infections and53(27.89%)were multiple infections.The difference between low CIN1and high CIN2/3single and multiple infections was statistically significant(χ^2=5.529,P=0.019).3HPV genotyping results,CIN1is mainly HPV-11,HPV-16,HPV-6,HPV-52,HPV-58infection,CIN2/3is mainly HPV-16,HPV-52,HPV-58,HPV-31,HPV-33infection,high-risk HPV infection is significantly associated with CIN disease,but its associated HPV high-risk subtypes are different.4CIN1HPV infection showed two peaks at25~29years old and35~39years old,and CIN2/3HPV infection was the highest at45~49years old,the difference was statistically significant(χ^2=41.148,38.071;P=0.000).Conclusion The peak of CIN1HPV infection in Meizhou area is25~29years old and35~39years old.The peak of CIN2/3HPV infection is45~49years old.HPV-16is the main pathogenic subtype.Infection with high risk HPV can increase the incidence of CIN2/3opportunity.
