摘要
目的:探讨代谢综合征(MS)及其组分对肺通气功能的影响。方法:将127例研究对象按MS组分个数不同分为4组:无代谢异常为正常对照组,1个组分代谢异常为代谢异常Ⅰ组,2个组分代谢异常为代谢异常Ⅱ组,≥3个组分代谢异常为代谢异常Ⅲ组(MS组)。所有研究对象均测量腰围(WC)、体重指数(Ibm)、血压,检测血糖、血肌酐(Cr)、血尿酸(UA),检查肺通气功能以及超声检查有无颈动脉硬化、脂肪肝等,比较各组以上各项指标的差异,应用多元线性回归模型分析MS与肺通气功能的关系。结果:4组肺活量(VC)、用力呼气肺活量(FVC)、1s用力呼气容积(FEV1)、用力呼出75%肺活量的呼气流量(MEF75%)和最大自主通气量(MVV)取实测值占预计值的百分比(VC%pred、FVC%pred、FEV1%pred、MEF75%pred和MVV%pred)及FEV1/FVC比较,差异有统计学意义(P<0.05)。其中MS组VC%pred、FVC%pred、FEV1%pred、FEV1/FVC、MVV%pred显著低于正常对照组(P<0.05),MS组VC%pred、FVC%pred、FEV1%pred、MEF75%pred、MVV%pred显著低于代谢异常Ⅰ组(P<0.05)。多元线性回归模型显示,随着WC、收缩压升高,Ibm的下降,FVC%pred呈下降趋势(β=-0.534、P=0.011;β=-0.221、P=0.043;β=1.674、P=0.004)。随着Ibm下降、WC的增加,FEV1%pred呈下降趋势(β=1.185、P=0.034;β=-0.495、P=0.014)。结论:MS患者肺通气功能较正常人下降,表现为混合性通气功能障碍,且肺通气功能障碍主要与MS组分中的高血压、中心型肥胖有关。
Objective:To investigate the effects of metabolic syndrome(MS)and its components on pulmonary ventilation function.Methods:A total of127subjects were divided into four groups according to the number of abnormal metabolic disorders:control group(no metabolic abnormality),metabolic abnormality groupⅠ(one component of metabolic abnormality),metabolic abnormality groupⅡ(two components)and metabolic abnormality groupⅢ(MS group,≥3components).The blood pressure,waist circumference(WC),height,weight,blood glucose,creatinine,uric acid and pulmonary function were measured.The ultrasound detection was performed to determine whether carotid atherosclerosis or fatty liver exists.The relationship between MS and pulmonary ventilation function was analyzed by multivariate analysis.Results:There were significant differences in VC%pred,FVC%pred,FEV1%pred,FEV1/FVC,MEF75%pred,and MVV%pred among the four groups(P<0.05).The VC%pred,FVC%pred,FEV1%pred,FEV1/FVC and MVV%pred in the MS group were significantly lower than those in the control group(P<0.05).The VC%pred,FVC%pred,FEV1%pred,MEF75%pred and MVV%pred in the MS group were significantly lower than those in the metabolic abnormality groupⅠ(P<0.05).Multiple linear regression model showed that the WC and systolic blood pressure increased and body mass index decreased with the FVC%pred decreased,and the body mass index decreased and WC increased with the FEV1%pred decreased(P<0.05).Conclusion:The pulmonary ventilation function of MS patients was reduced,which was characterized by mixed ventilation dysfunction.Moreover,the dysfunction of pulmonary ventilation was mainly related to blood pressure and central obesity of the MS components.
