摘要
目的分析龙泉市城镇社区糖尿病视网膜病变(diabetic retinitis,DR)患病率及危险因素,为预防和控制该地区DR发生提供科学依据。方法对2016年6月至2017年12月到龙泉市龙渊街道、塔石街道社区卫生服务中心和龙泉市人民医院住院治疗的城镇居民中糖尿病患者进行调查,利用Logistic回归模型进行危险因素分析。结果糖尿病视网膜病变患病率为24.7%,DR组的糖尿病病程比非DR组长(t=7837,P<0.001),饮酒者DR患病率高于不饮酒的人(χ~2=3.970,P=0.042),经常运动的糖尿病患者患DR率低于不运动的人(χ~2=11.312,P=0.001),有高血压史、糖尿病家族史的糖尿病患者DR患病率高(P<0.05),DR组DBP、FPG、胰岛素水平、2 h胰岛素水平、HbA1C、肌酐、尿酸、尿清蛋白水平均高于非DR组(P<0.001),HDL水平低于非DR组(P<0.001);多因素Logistic回归显示糖尿病病程长、空腹血糖高、HbA1C高和有糖尿病家族史的人患DR风险高;经常运动的人患DR风险更低。结论 FBG、糖尿病病程、糖尿病家族史、HbA1C高以及缺乏运动是糖尿病患者发生视网膜病变的独立危险因素。
Objective To understand the prevalence and risk factors of diabetic retinopathy(DR)in urban communities if Longquan so as to obtain scientific knowledge to better inform DR prevention and control practitioners of the area.Methods Firstly,a survey was conducted on the patients who were admitted to Longquan Longyuan Street Community Health Service Center,Stone Tower Street Community Health Service Center and the People’s Hospital of Longquan and diagnosed as cases of DR during the period from June 2016 to December 2017.Logistics regression model was then used for risk factors analysis.Results The rate of prevalence of diabetic retinopathy was 24.7%.The duration of diabetes in the DR group was longer than that in non-DR group(t=7837,P<0.001).The rate of prevalence of DR in alcoholic drinkers was greater than that in non-drinkers(χ^2=3.970,P=0.042).The rate of prevalence of DR in diabetics doing regular physical exercise was lower than that in those who did go in for physical exercise(χ^2=11.312,P=0.001).The greatest rate of prevalence of DR was found in those who were from families with history of diabetes and had history of hypertension(P<0.001).The levels of DBP,FBG,insulin,2h insulin,HbA1c,creatinine,uric acid,urinary albumin,and HDL of the DR group were significantly higher than those in the non-DR group(P<0.001).Multi-factor Logistics regression showed that those who had longer duration of diabetes,higher level of fasting blood glucose,HbA1c and were from families with history of diabetes had higher risk of suffering DR.Those who regularly went in for physical exercise were confronted with lower risk of having DR.Conclusions FBG,duration of diabetes,family history of diabetes,HbA1c,and lack of physical exercise are independent risk factors of retinopathy in diabetic patients.
