摘要
目的探讨基质金属蛋白酶-2(matrix metalloproteinase,MMP-2)在非小细胞肺癌(non small cell lung cancer,NSCLC)血清及癌组织中的表达及其与转移和预后的关系。方法 (1)用ELISA法检测85例NSCLC患者和20例肺良性病变患者血清中MMP-2的表达;(2)用免疫组织化学方法检测85例NSCLC患者和20例肺良性病变患者组织中MMP-2的表达;(3) NSCLC患者术后5年生存期调查。结果 (1)肺癌组(实验组)术前血清MMP-2水平(529. 23±57. 16) ng/m L显著高于肺癌组术后(260. 45±24. 21) ng/m L和对照组(242. 16±30. 43) ng/m L(P <0. 05);肺癌组(实验组)术前血清MMP-2水平与肿瘤直径大小、肿瘤病理类型、肿瘤临床分期、以及淋巴结转移情况有显著相关性(P <0. 05);(2)肺癌组(实验组)癌组织中MMP-2阳性率为55. 29%,其表达与肿瘤直径大小、病理类型、临床分期、是否有淋巴结转移有明显相关性(P <0. 05);(3)肺癌组MMP-2蛋白表达阳性者其血清MMP-2水平显著高于肺癌组MMP-2蛋白表达阴性者的血清MMP-2水平(P <0. 05);(4)血清高表达MMP-2肺癌患者术后5年生存率(2. 33%)显著低于低表达MMP-2肺癌患者(7. 14%)(P <0. 01)。结论血清中MMP-2水平与癌组织中MMP-2蛋白的表达基本一致,MMP-2可能在肿瘤浸润和转移过程中起着促进作用。
Objective To explore the expression of MMP-2 in serum and tissues of NSCLC and its relationship with metastasis and prognosis.Methods (1)The expression of MMP-2 in serum was detected in 85 NSCLC patients and 20 patients with benign pulmonary lesions by ELISA.(2)The expression of MMP-2 in tissues was detected in 85 NSCLC patients and 20 patients with benign pulmonary lesions by immunochemistry.(3)5-year survival of patients with NSCLC after operation was investigated.Results (1)The serum level of MMP-2 in NSCLC patients was significantly higher before treatment(529.23±57.16)ng/mL than after(260.45±24.21)ng/mL and in the control group(242.16±30.43)ng/mL(P<0.05).Before treatment,serum MMP-2 level was closely related with tumor diameter,pathological type of tumor,clinical stage of tumor and lymph node metastasis in NSCLC patients(P<0.05).(2)The positive expression rate of MMP-2 was 55.29%in NSCLC tissues,and it was closely related with tumor diameter,pathological type,clinical stage and lymph node metastasis(P<0.05).(3)The serum level of MMP-2 was significantly higher in the positive expression of MMP-2 protein group than in the negative group(P<0.05).(4)The postoperative 5-year survival rate of lung cancer patients with high expression of MMP-2(2.33%) was significantly lower than that patients with low expression of MMP-2(7.14%)(P<0.01).Conclusion The level of MMP-2 in serum is consistent with the expression of MMP-2 protein in cancer tissue.MMP-2 can be taken as an important part in the process of tumor invasion and metastasis.
